Job summary
Angel Hill Surgery has a fantastic opportunity for an experienced practice nurse to join our supportive nursing team, on a part time basis for 19 hours per week. We are a friendly practice, of over 14,000 patients with 5 GP partners and 5 salaried GPs.
Main duties of the job
- Undertaking health assessments, administering medication and treatments, and performing diagnostic tests.
- Providing and evaluating a range of health interventions to ensure that patients health and social care needs are met.
- General health and well-being promotion as well as creating individual, person-centred plans for patients with, or at risk of, a long term condition.
- Managing and treating long-term conditions, in line with national and local policies and the practice needs.
- Reviewing medication for therapeutic effectiveness, in accordance with evidence-based practice as well as national and practice protocols.
- Prioritising health problems and intervening appropriately.
- Supporting and managing the health needs of women presenting for sexual health and contraception, and cervical cytology consultations.
- Implementing, and participating in, vaccination and immunisation programmes for both adults and children.
- Meeting the needs of patients presenting for opportunistic wound care.
- Hypertension review and management.
- Running asthma and COPD clinics and reviews.
