Data and Outreach Coordinator

Job summary

Rochford District Primary Care Network (PCN) have a fantastic opportunity for a permanent data and outreach coordinator to join our team. This position is offered on a full-time basis, working 37.5 hours per week, ideally Monday to Friday and is covering the following practices:

• Ashingdon Medical Centre
• The Practice Leecon Way
• The Puzey Family Practice
• Wakering Medical Centre

The role involves providing high quality, comprehensive and effective administration skills, whilst providing a central co-ordination role for patient care planning across member practices.

Main duties of the job

The appropriate individual will have SystmOne and primary care experience.

You will need to be confident individual who can communicate with a ride range of professionals at all levels.

The successful applicant will have the ability to work without supervision using their own initiative, organising and prioritising own and other workloads to changing and often tight deadlines.

You will be enthusiastic and committed.

Rochford PCN priorities include:

• Addressing health inequalities at a local level by using data & local intelligence through partnership with local Wellbeing Link Coordinators to reduce social isolation. We aim to do this by promoting the local groups & encouraging individuals to share their skills.
• Enhancing our staff & wellbeing by upskilling our local workforce to make general practice an attractive place to work. Supporting our staff to have access to relevant training & development opportunities so that they are continuing to improve the care that they are providing to patients.
