Job summary

Sheepcot Medical Centre has a fantastic opportunity for an experienced nurse practitionerto join our team on a part-time basis, for 25-30 hours per week.

Main duties of the job

  • Undertaking health assessments, administering medication and treatments, and performing diagnostic tests.
  • Providing and evaluating a range of health interventions to ensure that patients health and social care needs are met.
  • General health and well-being promotion as well as creating individual, person-centred plans for patients with, or at risk of, a long term condition.
  • Managing and treating long-term conditions, in line with national and local policies and the practice needs.
  • Reviewing medication for therapeutic effectiveness, in accordance with evidence-based practice as well as national and practice protocols; working with patients to support their adherence to prescribed treatments.
  • Prioritising health problems and intervening appropriately, to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.
  • Supporting and managing the health needs of women presenting for sexual health and contraception, and cervical cytology consultations.
  • Implementing, and participating in, vaccination and immunisation programmes for both adults and children, including patients travelling abroad.
  • Meeting the needs of patients presenting for opportunistic wound care.
  • Hypertension review and management.
  • Running asthma, COPD and diabetes clinics and reviews.
  • Home visits to our housebound patients for their long term condition reviews
  • New patient and health checks.

About us

We are a three-partner practice with approx. 11,000 patients based in a modern purpose built premises. We have four salaried GPs, 3 practice nurses, 1 HCA, and a variety of ARRS workers that support our practice and PCN including paramedics, advanced nurse practitioners, mental health workers and clinical pharmacists. We are a huge training practice, working alongside multiple medical schools and we pride ourselves on providing the very best placements. Teaching, learning and education development are at the heart of our core values. We feel that it is very important to general practice that we encourage and show the students the importance of general practice and hopefully we are helping provide the NHS with the next generation of clinicians.

  • Competence to undertake asthma, COPD and diabetic clinics and reviews, holding relevant certificates.
  • Evidence of clinical skills and experience in treatment room consultation, phlebotomy, ECG recording, screening, management of long-term conditions, cervical cytology sampling, vaccinations and immunisations, dressing and wound care, and recording spirometry.
  • Compassionate manner with an empathetic approach and demonstrable kindness in all aspects of your work. The successful candidate will value the dignity of our patients, their relatives and carers, as well as their colleagues at the practice.
  • Knowledge, and experience of its application, in supporting long-term conditions.
  • Insight and understanding of current issues in nursing and the wider NHS.
