Nurse Practitioner

Job summary

Sheepcot Medical Centre has a fantastic opportunity for an experienced nurse practitionerto join our team on a part-time basis, for 25-30 hours per week.

Main duties of the job

• Undertaking health assessments, administering medication and treatments, and performing diagnostic tests.
• Providing and evaluating a range of health interventions to ensure that patients health and social care needs are met.
• General health and well-being promotion as well as creating individual, person-centred plans for patients with, or at risk of, a long term condition.
• Managing and treating long-term conditions, in line with national and local policies and the practice needs.
• Reviewing medication for therapeutic effectiveness, in accordance with evidence-based practice as well as national and practice protocols; working with patients to support their adherence to prescribed treatments.
• Prioritising health problems and intervening appropriately, to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.
• Supporting and managing the health needs of women presenting for sexual health and contraception, and cervical cytology consultations.
• Implementing, and participating in, vaccination and immunisation programmes for both adults and children, including patients travelling abroad.
• Meeting the needs of patients presenting for opportunistic wound care.
• Hypertension review and management.
• Running asthma, COPD and diabetes clinics and reviews.
• Home visits to our housebound patients for their long term condition reviews
• New patient and health checks.

We are a three-partner practice with approx. 11,000 patients based in a modern purpose built premises. We have four salaried GPs, 3 practice nurses, 1 HCA, and a variety of ARRS workers that support our practice and PCN including paramedics, advanced nurse practitioners, mental health workers and clinical pharmacists. We are a huge training practice, working alongside multiple medical schools and we pride ourselves on providing the very best placements. Teaching, learning and education development are at the heart of our core values.
