Job summary

Deben Health Group has an exciting opportunity for a hardworking & enthusiastic PCN prescribing nurse practitioner/ paramedic, on a full or part time basis. This new role would join an established PCN team of patient coordinators, mental health, pharmacy hub & first contact physiotherapists. You will be required to travel across the sites within the Deben Health Group PCN which is made up of 4 practices.

Main duties of the job

  • Assess & triage patients, including same day triage.
  • Advise patients on general health and promote self-management where appropriate, including signposting patients to the PCNs social prescribing service, and where appropriate, other community or voluntary services.
  • Perform and interpret ECGs.
  • Perform investigatory procedures, specialist health checks and QOF investigations as required within their scope of practice and in line with local and national guidance.
  • Undertake the collection of pathological specimens.
  • Support the delivery of anticipatory care plans & lead certain community services.
  • Provide an alternative model to urgent and same day GP home visit for the network & clinical audits.
  • Communicate proactively and effectively with all colleagues across the multi-disciplinary team, attending and contributing to meetings as required.
  • Support the PCN care-home population by providing medical services (ward rounds, urgent care visits where necessary).

Qualifications

Essential

  • Have current HCPC registration and have post-registration experience, ideally with some being in primary care.
  • Clinical skills qualifications or evidence of equivalent experience.
  • Minimum of 2 years (or equivalent) post qualification experience as a newly qualified paramedic.
  • 3 years experience or equivalent as a band 6 or equivalent paramedic.

  • Advanced clinical practice skills.
  • Be able to effectively assess, plan, treat and refer patients.
  • Be able to evaluate patient care confidently.
  • Experience in the management of long term conditions.
  • To exhibit excellent verbal and non-verbal communication skills.
  • Have the ability to make clear decisions with confidence and communicate these effectively.

Desirable

  • Experience in management of patients with complex needs.
  • Experience of preventative medicine and chronic disease management.
  • Experience of working to achieve standards within the Quality and Outcome Framework (QOF)
  • Recent experience of working in UK general practice environment.
  • 2 years experience or equivalent of working as an emergency care practitioner in one of the following: General Practice, WIC/MIU, GP OOHs or A&E.
  • 5 years post registration experience or equivalent of which 2 years will have been spent in primary care, A&E unscheduled care or related field.
