Job summary

Sidney House & The Laurels Surgery is looking to welcome a qualified and experienced clinical pharmacist into our practice, to play a full and active role within our multi-disciplinary team. The role is offered on a full or part-time basis and the successful candidate will be required to work some of their hours as part of the Extended Access Scheme i.e. between 18:30-20:00 Monday to Friday, or on a Saturday however these hours could be worked remotely. One of the key responsibilities of this role will be to carry out chronic disease reviews, supported by our nursing team.

We are keen to hear from experienced pharmacists, particularly with experience working in general practice.

Main duties of the job

  • Monitoring patients with complex, long-term conditions such as hypertension, diabetes and COPD, ensuring that medication is optimised, titrated, monitored and complied with.
  • Clinically assessing and treating patients, using expert knowledge of medicines, for specific disease areas.
  • Providing specialist expertise in the use of medicines and medicine information (e.g. side-effects, dosage).
  • Providing pharmaceutical medication review and health education advice to patients, by face-to-face, telephone or notes-based review, as agreed with the practice.
  • Supporting the practice dispensary.
  • Providing clinical advice and expertise on medication.
  • Managing the repeat prescribing reauthorisation process, by reviewing requests for repeat prescriptions & medicines reaching review dates.
  • Carrying out structured medicines-optimisation reviews in our care homes.

About us

We are a two-site training practice based in the semi-rural villages of Hatfield Peverel and Boreham, serving a patient population of 11,200. We are a SystmOne Practice with a consistently high QOF achievement and a CQC rating of Good.

We are part of the Aegros PCN with Lt. Waltham & Great Notley, Douglas Grove Surgery and the Beacon Health Group.

Our team is made up of three GP partners, five salaried GPs, a paramedic practitioner, a nurse prescriber, practice nurse, two HCAs, a phlebotomist and dispensers. We are further supported by PCN staff including a pharmacist and physiotherapist, additionally we have a well established non-clinical team of managers, administrators, dispensers and receptionists.
