Job summary
EAST:MK PCN has an exciting opportunity for an enhanced nurse to join our practice team on a full-time basis, for 37.5 hours per week over 5 days, between Monday & Saturday, on a rotation basis across the PCN's 4 practices.
We are seeking an enthusiastic person who excels at providing the best possible patient care, working as a self-motivated individual and as part of a multi disciplinary team.
Main duties of the job
- Consult with patients, their family & the multi professional team to undertake assessments of patient need, devising & evaluating complex care plans.
- Evaluate & analyse clinical problems using clinical knowledge, seeking out & applying relevant evidence, enhanced technique interventions & equipment to make clinical decisions.
- Deliver enhanced clinical care using analysis & own knowledge to manage complex interventions.
- Teach & advise the patient & their families how to manage their long-term condition or support the multi disciplinary team to do so.
- Facilitate & support the development & delivery of innovative training & education to staff, to improve competence & confidence.
- Work proactively with key local & national stakeholders to develop integrated pathways & holistic models of care.
- Provide information & advice about a wide range of services in order to promote health & well-being in order to support independence within communities.

EAST:MK PCN covers a total population of approximately 55,000 patients, across its 4 practices in Milton Keynes: Ashfield Medical Centre, CMK Medical Centre, Milton Keynes Village Practice and the Grove Surgery.

Qualifications
Essential
- Hold a degree in nursing.
- Registered with the NMC.
- Hold a post graduate qualification at level 7 or above, ideally in complex wound dressings, diabetes, respiratory conditions and CVD, dementia, public health or population health.
- Independent prescribing V300.
Experience
Essential
- Experience of nursing in care home settings.
- Extensive experience of minor illness.
- Strong listening and communication skills.
- You will be familiar with local resources and services and how to access them.
- Motivated by helping people, with care and empathy.
- Ability to deliver person-centred support in a non-judgmental way.
- Experience of completing holistic, person-centred care-planning assessments.
- Understanding of the needs of small, volunteer-led community groups and the ability to support their development.
- Commitment to collaborative working with all local agencies.
- Experience and confidence in the delivery of lifestyle change interventions.
