PCN Pharmacy Technician

Job summary

Deben Health Group PCN are looking for a pharmacy technician on a part time basis for 20 hours per week.

Main duties of the job

  • Work with the clinical pharmacists and clinicians in accordance with agreed practice prescribing standard operating procedures
  • As determined by the PCN, support medication reviews/monitoring and medicines reconciliation post hospital discharge (or other transfer of care) and synchronising medicines for patient
  • Support the clinical pharmacist in Structure Medication Reviews (SMR) i.e. organise necessary monitoring tests prior to SMR
  • Assist the PCN in achieving targets within the PCN DES and IIF contract.
  • Ensure effective medicines use, through shared decision-making conversations with patients
  • Lead improvements to maximise safe, cost effective best practice in prescribing to improve the quality of patient care
  • Support patients to get the best from their medicines by switching medications to agreed and approved protocols
  • Improve repeat prescribing processes within PCN, to include promotion of repeat dispensing and online ordering, minimising clinical risk and aiming to reduce wasted medicines
  • Assist the PCN in achieving national requirements, NICE implementation and utilisation of medicines optimisation initiatives
  • Undertake patient supporting roles to ensure effective medicines use, through shared decision-making conversations with patients

The PCN is made up of four GP practices working in collaboration, based on the Suffolk Coast, providing a positive patient experience.

Qualifications

Essential

  • Professional registration with GPhC
  • BTEC/NCQ level 3 or equivalent in Pharmaceutical sciences

Experience

Essential

  • Have at least 2 years of experience (or equivalent) as a pharmacy technician within a hospital, community or primary care setting.
  • Experience of dealing with patients face to face in clinics as well as over the telephone and digitally
  • Experience of working within a diverse team with multiple disciplinarians
  • Excellent communication skills, verbal and written, with the ability to adjust communication style and content to suit the audience
  • Should be enrolled in, or willing to undertake an approved training pathway. (PCPEP/CPPE)
  • Sound computer skills, proficient with MS Office
  • System One experience preferred
  • An excellent understanding of data protection and confidentiality issues
  • Has attention to detail, able to work accurately, identifying errors quickly and easily
  • Self-motivated and proactive
