Job summary

We are looking for an experienced and patient focused clinical pharmacist to join our team on a 2 year fixed term contract. The successful candidate will work across both our sites and whilst the position is ideally full time, we will consider candidates looking for a part-time role.

Main duties of the job

  • Conduct medication reviews and provide expert clinical advice to patients.
  • Collaborate with GPs, nurses, and other healthcare professionals to optimise patient care.
  • Manage repeat prescriptions and ensure safe and effective use of medications.
  • Support patients with chronic conditions and promote health and well-being.
  • Participate in clinical audits and quality improvement initiatives.

We are a friendly and patient-focused GP organisation located in Norwich and operating Bacon Road Medical Centre and Taverham Surgery. Our team is dedicated to providing high-quality healthcare services to our community. The team comprises of:

  • An existing clinical pharmacist who will be a valuable resource for the new team member.
  • A trainee pharmacy technician who is eager to learn and contribute to patient care.
  • Staff qualified to dispensary assistant level 2 to provide essential support.
  • Our team is further supported by a West Norwich PCN clinical pharmacist.

Qualifications

Essential

  • Completion of a degree in pharmacy and registration with the General Pharmaceutical Council;
  • Post-qualification experience in pharmacy (hospital, primary care or community), or working towards obtaining this;
  • Qualified independent prescriber, or working towards this;
  • Member of the Royal Pharmaceutical Society (RPS);
  • The clinical pharmacist is enrolled in, or has qualified from, an approved 18-month training pathway or equivalent that equips them to:
  • a. Be able to practice and prescribe safely and effectively in a primary care setting (for example, the CPPE clinical pharmacist training pathways)
  • b. Although optional for organisations other than a PCN, it is thought advisable that they are able to deliver the key responsibilities outlined in section B1.3 of the Network Contracted Enhanced Service Specification 2023-24.

Experience

Essential

  • Awareness of the breadth of common acute and long-term conditions that are likely to be seen in a general medical practice;
  • Ability to demonstrate general pharmaceutical skills and knowledge in core areas;
  • Ability to plan, manage, monitor, advise and review general care plans for patients with long-term conditions;
  • Ability to demonstrate accountability for delivering professional expertise and direct service provision as an individuals;
  • Ability to recognise priorities when problem-solving and identify deviations from normal patterns, referring to seniors or GPs with confidence, when appropriate;
  • Ability to follow legal, ethical, professional and organisational policies, procedures and codes of conduct;
  • Ability to assist patients in decisions about prescribed medicines and supporting adherence as per NICE guidance;
  • Non-judgemental attitude and ability to be flexible and adaptable;
  • Self-motivated with excellent communication skills;
  • Understand the need for patient confidentiality.
