Job summary
Langdon Hills Medical Centre is looking to welcome a qualified and experienced clinical pharmacist into our busy practice, to join our growing, dynamic four partner training practice in Basildon. The role is offered on a part-time basis for 20 hours per week. We are looking for a friendly, enthusiastic and highly motivated practitioner who enjoys working as part of a multi-disciplinary team.
Main duties of the job
The post holder will work within clinical competencies as part of a multidisciplinary team, to provide expertise in medicines management as follows:
- Provide structured medication reviews;
- Manage long term conditions, medicines on transfer of care and systems for safer prescribing, repeat authorisation and acute prescription requests.
- Ensure that medicines management processes within the Practice are optimised to achieve highly effective, safe and patient centred prescribing.
- Providing leadership on person-centred medicines optimisation and quality improvement;
- Contributing to public health campaigns, including flu vaccinations, childhood immunisation and adult immunisation programmes;
- Supporting improvements in clinical care, through practice-based audits and implementing change;
- Undertaking minor ailments triage and directing patients, based on clinical need;
- Monitoring patients with complex, long-term conditions such as hypertension, diabetesand COPD, ensuring that medication is optimised, titrated, monitored and complied with;
- Taking a central role in the clinical aspects of shared-care protocols, clinical research withmedicines and liaison with specialist pharmacists;
- Being part of a professional clinical network and having access to appropriate clinicalsupervision.
