Job summary
Larksfield and Arlesey Medical practice has a fantastic opportunity for an experienced or newly qualified practice nurse to join our team on a full-time basis for 37.5 hours per week.
Main duties of the job
- Undertaking health assessments, administering medication and treatments, and performing diagnostic tests.
- Providing and evaluating a range of health interventions to ensure that patients health and social care needs are met.
- General health and well-being promotion as well as creating individual, person-centred plans for patients with, or at risk of, a long term condition.
- Managing and treating long-term conditions, in line with national and local policies and the practice needs.
- Reviewing medication for therapeutic effectiveness, in accordance with evidence-based practice as well as national and practice protocols; working with patients to support their adherence to prescribed treatments.
- Prioritising health problems and intervening appropriately, to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.
- Supporting and managing the health needs of women presenting for sexual health and contraception, and cervical cytology consultations.
- Implementing, and participating in, vaccination and immunisation programmes for both adults and children, including patients travelling abroad.
- Meeting the needs of patients presenting for opportunistic wound care.
- Hypertension review and management.
- Running asthma, COPD and diabetes clinics and reviews.
- New patient and health checks.
About us
We have a patient population of 19,500 patients across our main site Larksfield and new branch site Arlesey medical centre. Both practices are now under new partnership and a new management team. Our current team consists of salaried GPs, an excellent management and administration team with a highly skilled MDT team including nurse practitioners, paramedic practitioners, practice nursing team and HCAs. We also have additional support from ARRS roles from the primary care network.
