Job summary

HertsOne Ltd has an exciting new post as part of a new initiative. We are looking to recruit a clinical pharmacist on either a full or part time basis, for a 12 month fixed term contract, who will work within the West Herts Enhanced Primary Care Monitoring service. The purpose of the role is to provide support and intervention to children and young people with ADHD (as well as co-morbidities) between the ages of 6 and 18.

You will be responsible for completing ADHD medication reviews utilising specialist skills and using clinical interview techniques to ascertain a child's response to medication and manage potential side effects. They will be expected to be confident in ascertaining potential symptoms of co-morbidity such as ASD, mental health needs or physical health concerns and document information accurately and refer to appropriate local services.

You will consider medication changes that may be required to the child/young persons medication care plan and discuss these with the supervising GP and relaying these care plans to the patients records.

Main duties of the job

      • Promote patient and public involvement activities in the specialist area, by involving patients, parents/relatives in the planning and delivery of care and development of services leading to service improvement.
      • Contribute to the development of leaflets and information regarding medication and ADHD for children, families, and professionals.
      • Ensure the effective and efficient use of physical and financial resources.
      • Collect, collate, evaluate, and report information and maintaining accurate patient records.
      • Ensure the service is monitored on a regular basis and data is available as a record to provide statistical information to Clinical Leads and others and to provide data to relevant local and national bodies to audit service as appropriate.

Experience

Essential

  • Knowledge of CYP legislation and relevant mental health legislation;
  • Knowledge of children's mental health and child development;
  • Understanding of theoretical frameworks that underpin the understanding of behaviour;
  • Ability to demonstrate general pharmaceutical skills and knowledge in core areas;
  • Ability to recognise priorities when problem-solving and identify deviations from normal patterns, referring to seniors or GPs with confidence, when appropriate;
  • Ability to follow legal, ethical, professional and organisational policies, procedures and codes of conduct;
  • Ability to assist patients in decisions about prescribed medicines and supporting adherence as per NICE guidance;
  • Non-judgemental attitude and ability to be flexible and adaptable;
  • Self-motivated with excellent communication skills;
  • Understand the need for patient confidentiality.

Qualifications

  • Completion of a degree in pharmacy and registration with the General Pharmaceutical Council.
  • Masters degree in pharmacy (MPharm) or equivalent.
  • Qualified independent prescriber.
  • Safeguarding Children & Adult Level 3.
