Job summary
We are seeking an enthusiastic clinical pharmacist to work on a full or part-time basis.
We are a friendly surgery with a list size of 16,000 patients, over two sites, Wootton Vale Health Centre, (MK43 9JJ), and branch site Shortstown Surgery, (MK42 0GS). Both practices are based on the outskirts of Bedford town centre and have easy access and free on-site car parking. Both practices share the same clinical and management teams and are active members of our PCN (Caritas). This role is pivotal to improving the quality of care and operational efficiencies & so, requires motivation & passion to deliver an excellent service within general practice.
We have good appointment access, which means happy patients, and a strong teamwork ethos that includes an established clinical pharmacy team, GPs, nurses, physician associates and a clinical liaison team who look after the clinical staff.
We support staff development and wellbeing, and we have several members of staff on diploma programmes or clinical leads in BLMK. We will support and encourage you to build on your LTC skills to attain level 7 diploma status. The ideal candidate will have already achieved a non-medical prescribing qualification and CPPE.
We embrace IT and automation and often pilot new innovations. We are an accredited GP training practice for students, apprentices, pre reg and GP trainees.
Training on local services will be provided.
Main duties of the job
The post holder will work within their clinical competencies as part of a multi-disciplinary team to provide expertise in the following areas:
- Clinical medicines management, provide face to face/remote structured medication reviews
- Management of long-term conditions asthma/COPD, CVD, diabetes, dementia/frailty
- Medicines optimisation, review of monitoring needs and blood interpretation
- Management of medicines on transfer of care and systems for safer prescribing
- Manage prescription requests including authorisations and reauthorisations
- Medicines safety and quality improvement, incorporating the use of national tools
- Ensure both the public health and social care needs of patients within the GP practices
- Regular audits to improve and influence prescribing across the practice
- Support and supervise other staff in undergoing training.

We are 2 practices who share the same clinical and management teams. We understand and value the expertise of our clinical pharmacists. Our ethos is to embrace new ways of working and workforce development & we have been involved in numerous early adopter and NHS pilots developing roles. We also work closely with our CCG in supporting quality & development in areas such as IT and medicines management.
The practices have a combined list size of 16,000. The two practice lists vary greatly in terms of demographic and depravation scores, thus providing a diverse, interesting mix of clinical challenge & will therefore appeal to clinicians looking to broaden their range of exposure & experience.
The team comprises: A clinical director (GP partner), 3 salaried GPs, 5 associate GPs, 3 nurses, women's healthcare specialist, HCA, senior clinical pharmacist, clinical pharmacist, pharmacy technician, 2 care coordinators, business director, practice manager and 16 admin staff.
