Job summary

Main duties of the job
- Provide assessment, diagnosis & treatment, at first point of contact, by attending to patients according to their needs in the practice
- Act as an autonomous clinician, with support & guidance from the GPs
- Proactively identify, diagnose & manage treatment plans for patients at risk of developing a long-term condition (as appropriate)
- Diagnose & manage both acute & chronic conditions, integrating both drug & non-drug based treatment methods into a management plan
- Accurately triage & prioritise patients, including in emergency situations, demonstrating the use of a variety of techniques to elicit the history of an event/illness, including past medical & drug history
- Recognise, assess & refer patients presenting with mental health needs
- Telephone triage as appropriate for clinics & home visits, providing access to GPs as necessary
- Evaluate clinical information from examination & history-taking, initiating appropriate treatment and/or referral, including admission to hospital if required
