Job summary

Leighton Linslade Health Connections has an exciting opportunity for a hardworking, enthusiastic and innovative pharmacy technician to join our primary care network multidisciplinary healthcare team, on a full-time basis, for 37.5 hours per week, to lead improvements to maximise safe, cost-effective best practice in prescribing to improve the quality of patient care.

You will be helping patients to get the best from their medicines by switching medications to agreed and approved protocols, improving repeat prescribing processes in general practice, including promotion of repeat dispensing and online ordering, minimising clinical risk, and aiming to reduce wasted medicines.

In addition, you will be responsible for encouraging the development of better understanding of the principles of medicines optimisation throughout the practice teams and promoting good practice in line with therapeutic developments.

Main duties of the job

The main responsibilities of the role include:

  • Undertaking patient facing & patient supporting roles to ensure effective medicines use
  • Carrying out medicines optimisation tasks including effective medicine administration, supporting medication reviews & medicines reconciliation
  • Supporting clinical pharmacists
  • Providing expertise to address both the public health & social care needs of patients, including lifestyle advice, service information & help in tackling local health inequalities
  • Managing shared care protocols & liaise with clinical pharmacists
  • Supporting initiatives for antimicrobial stewardship to reduce inappropriate antibiotic prescribing
  • Supporting the PCN multi-disciplinary team to ensure efficient medicines optimisation processes are being followed
  • Implementing efficient ordering & return processes & reducing medication wastage
  • Providing training & supporting the legal, safe & secure handling of medicines, including the implementation of Proxy Access for Care Homes
  • Promoting of electronic Repeat Dispensing & online ordering
  • Developing relationships with other pharmacy professionals & members of the multi-disciplinary team supporting integration across health & social care including primary care, community pharmacy, secondary care & mental health
  • Supporting the implementation of national prescribing policies & guidance within GP practices, care homes & other primary care settings
  • Supporting the PCN to deliver on IIF, QOF & locally commissioned enhanced services
