Job summary

GP Healthcare Alliance (a federation of GP practices in Castle Point and Rochford) is working in partnership with the local primary care networks to support the delivery of the NHS England, Direct Enhanced Service (DES) contract.

Main duties of the job

We are looking to recruit a PCN transformation manager to join a wider team covering Canvey PCN. and you will be based at the PCN office in Canvey. This is a fixed-term contract for 12 months, to provide maternity cover. The start date is January 2024.

About us

GP Healthcare Alliance is a company originally formed as a GP Federation made up of 18 practices in Castle Point, Rochford and Rayleigh, Essex, working together to provide healthcare for all practices and patients.

All practice members remain independent contractors but work collaboratively to improve healthcare services within our area.We provide a range of primary care and community services to our local residents, various at scale services for practices as well as professional staff training. We meet nationwide challenges with local knowledge and expertise, tailoring our services to reflect the needs of the communities in which we work.

We will develop new services where we believe we can improve the patient experience and deliver efficiency savings to the NHS. We work with local NHS clinicians in the delivery of the services, thereby supporting the retention of strength and depth within the local NHS clinical teams.

Job responsibilities

  • Operational management of the primary care network in line with local and national guidelines published by NHS England.
  • Delivery of projects across the network and member practices, monitoring risks, presenting data with the impacts of these schemes.
  • Reviewing data to support population health management and workforce planning.
  • Working in partnership with the clinical team leaders to forward plan and arrange the protected learning sessions.
  • Develop strong relationships with their local PCN practices and will support the planning and delivery of improvements that will provide benefits to patients and staff.
  • Leading recruitment, retention, and management of the PCN workforce.
  • Take responsibility for ensuring adherence to programme budgets, ongoing monitoring of expenditure against budget and ensuring that appropriate documentation is available for scrutiny.

This role involves providing high quality, comprehensive and effective programme management skills.

Essential

  • Educated to degree level or demonstrate equivalent experience.
  • Excel advanced (or equivalent skills).
  • Experience of working in general practice with a good working knowledge of general practice systems (including IT, digital and BI tools), processes, and procedures.
  • Experience of researching and developing new services or significant service improvements.
  • Experience of successfully delivering change and improvement programmes in a patient (or customer) facing environment.
  • Experience of successful project management through the full life cycle. Experience of Systmone clinical system.
  • Proven ability to complete projects according to outlined scope, budget, and timeline.
  • Experience of developing and monitoring service KPIs.
  • Demonstrable experience of people management.
  • Experience of developing engagement and communication initiatives.
  • Experience of championing diversity.
  • Experience in a similar role.
  • Knowledge of Quality Improvement tools or a willingness to engage in the Quality Improvement training offers.
  • Experience of championing diversity and inclusion and promoting actions to make improvements to the experience of diverse groups, including promoting digital inclusion.
