Job summary

We have an exciting, new opportunity for a care co-ordinator to join the South Rural Primary Care Network. The role is based at the Constable Country Medical Practice.

Main duties of the job

The role involves working very closely with the practice and the multidisciplinary team (MDT) within the PCN. The role is pivotal in ensuring all patients receive the best possible care and service.

About us

South Rural PCN was formed in June 2019, with five like-minded practices wanting to provide a joined up service for local communities. To achieve this we are looking to reshape our clinical teams by developing new and exciting skill mixes, which not only diversify our workforce, but allows us to provide a better care package for our patients.

The PCN provides services to 56,000 patients across rural Suffolk and is headed by 2 proactive, supportive and committed Clinical Directors. Our PCN includes the following practices: Bildeston Health Centre, Constable Country Medical Practice (East Bergholt and Capel St Mary), Hadleigh Health Centre (Boxford), The Holbrook and Shotley Surgery and Needham Market Country Practice.

Our practices offer the full range of primary care services. We achieve consistently high QOF and CQC results. We offer extended hours appointments to our patients. We provide an enhanced health in care homes service to the 16 care homes across our area.

Essential

  • If you are a skilled care coordinator, with excellent person-centred planning experience, local knowledge of key support and advice available to our patient population, then we'd love to hear from you.
  • All applicants should have the following skills and qualifications:
  • Essential:
  • Good IT skills.
  • Experience of working within healthcare, the voluntary or community sector.
  • Experience supporting vulnerable groups.
  • Strong listening and communication skills.
  • Motivated by helping people, with care and empathy, in delivering person-centred support in a non-judgmental way.

Desirable

  • Diploma Level 2 in Health and Social Care or equivalent qualification.
  • Familiar with local resources and services and how to access them.
  • Experience of completing holistic person centred care planning assessments.
