WGGL PCN are looking to recruit a care coordinator to join our team, the post is for 20 hours per week and will be based at the Guildhall Surgery, Clare

Main duties of the job

The role involves working very closely with the practices and the multidisciplinary team (MDT) within the PCN, and is pivotal in ensuring that all patients receive the best possible care and service.

About us

The practices within the WGGL Primary Care Network collaboratively work together, are within close proximity and between them look after over 25,000 patients.

In this role you will act as part of a multi-professional NHS team in providing social prescribing services that will protect, maintain and restore the health of our patients.

Job description

Job responsibilities

The main duties of the role include:

  • Assist with personal care plans for individual patients, ensuring preventative actions are detailed to support the appropriate use of services.
  • To collate patients identified care and support needs and review the options to meet these needs and bring them into a single personalised care and support plan in line with best practice
  • Help people to manage their needs through answering queries, making and managing appointments, and ensuring that people have good quality written or verbal information to help them make choices about their care
  • To provide coordination and navigation of patients, and where appropriate their carers, across health and social care services.
  • To support in the delivery of enhanced services and other service requirements on behalf of the PCN
  • Support delivery of QOF, incentive schemes, QIPP and other quality or cost effectiveness initiatives
  • To run regular patient searches using the clinical IT system in order to have an up to date record of progress of achievement of Key Performance Indicators
  • Work closely with the practice / information and data manager to aid with annual patient recall systems
  • Document data on the computer system using clinical coding to ensure accurate retrieval of information for monitoring and audit purpose
  • Assist with the annual equipment check and calibration
  • Maintain recall system for national and annual health screening programs.
  • Recording data on Open Exeter or equivalent for national immunisation programmes
  • Assist the clinical team in following up patients who have home treatment care, ensuring that any issues are recorded and followed up with the clinical team
  • Assist in carry out clinical audits when required.
  • Complete any time sheets to record work done or correct times of arrival and departure to and from work each day as appropriate.
  • Attend practice meetings - as per practice meeting schedule. This may require you to be flexible about your hours in order to enable you to regularly attend these meetings at the request of management team.
  • To work in conjunction with the management team to develop and coordinate clinical rotas.
  • Participate in any practice development training sessions.
  • Identifying patients with possible Access Needs and recording on the record.
  • Comply with any new practice procedures for any new IT developments for the Electronic Patient Record.
  • Assisting with the organisation, planning and carrying out of annual vaccination campaigns.
  • Undertake any other duties requested by the Practice Management team or clinical staff.

  • If you are a skilled care co-ordinator, with excellent person-centred planning experience and strong local knowledge for accessing key support, then we would love to hear from you.
  • The ideal applicant will have have the following skills, experience and qualifications:
  • Experience of working within healthcare, the voluntary or community sector, to support vulnerable groups;
  • Ideally you will have a Diploma Level 2 in Health and Social Care, or an equivalent qualification;
  • Strong listening and communication skills;
  • Familiar with local resources and services, including how to access them;
  • Motivated by helping people, with care and empathy;
  • Able to deliver person-centred support in a non-judgmental way;
  • Experience of completing holistic person-centred care planning assessments.
