Main duties of the job

The successful candidate will work independently at network and practice level, but also as part of a supportive practice team, to deliver safe and effective use of medicines by applying their clinical and pharmaceutical skills to a variety of tasks within the medicines optimisation agenda.

Suthergrey House Medical Centre works within Central Watford PCN which consists of three practices and offers services to nearly 40k patients.

Job description

Job responsibilities

The Clinical Pharmacist is very much a patient-facing role and there will be plenty of opportunity to utilise your clinical skills to improve the health of the local community. This will include managing long-term conditions, supporting hospital discharge prescribing arrangements, providing specific advice for those on multiple medications and supporting the GP practices involved to develop the clinical pharmacist role. You will act as a specialist resource to answer medicines-related queries, to signpost to other services and to liaise with other healthcare professionals as required.

The main responsibilities of the role include:

  • Clinically assessing and treating patients, using expert knowledge of medicines, for specific disease areas.
  • Providing specialist expertise in the use of medicines and medicine information (i.e. side-effects, dosage) across the practice.
  • Providing pharmaceutical medication review, and health education advice, to patients at the designated practices by face-to-face, telephone or notes-based review, as agreed with the practice.
  • Providing leadership on person-centred medicines optimisation and quality improvement.
  • Contributing to public health campaigns, including flu vaccinations, childhood immunisation and adult immunisation programmes.
  • Processing specialist treatment letters.
  • Supporting practice dispensary.
  • Processing hospital medication discharge letters.
  • Acting as a key contact in the practice, for all medication related queries.
  • Supporting improvements in clinical care, through practice-based audits and implementing change.

You will have access to a senior Clinical Pharmacist and a GP for supervision. These are new, exciting posts in which you could make a real impact. As the role is patient-facing, you will see how your role directly impacts the community you serve. There will be further progression as per the needs of the locality population and practice need.

Essential

  • We are looking for candidates who are enthusiastic and adaptable to change, with excellent clinical skills and a good level of competence in use of IT. Candidates should exhibit good organisational skills and ability to communicate effectively with a wide range of healthcare professionals and other stakeholders including patients, families, and carers.
  • The ideal candidate will have the following skills:
  • Completion of Pharmacy degree and at least 3 years post-graduate experience.
  • Ideally will have Independent Prescriber status.
  • Excellent clinical and medication knowledge.
  • Understanding and experience of working as a pharmacist in a GP practice setting, with a confident approach.
  • Experience of undertaking medication reviews.
  • The commitment to continue to learn and develop.
  • The ability to be adaptable and flexible and be a proactive problem solver.
  • Excellent inter-personal patient facing communication skills.
  • Registered with the General Pharmaceutical Council.
  • Will hold Personal Professional Indemnity Insurance.
