Job summary

Parkfield Medical Centre is looking to welcome a qualified and experienced clinical pharmacist into our busy practice, to play a full and active role within our multi-disciplinary team. The role is offered on apart-time basis with hours to suit the successful applicant, on a fixed term contract until March 2024 but we hope the funding will be extended and we would look to extend the contract for 12 months/into a permanent role.

About us

The practice provides services to a patient population of 13,000. We have an excellent complement of administrative and nursing staff including; 1 matron 2 practice nurses, 1 HCA, 1 paramedic, 1 phlebotomist and a practice pharmacist. We are a high achieving surgery in all clinical and quality areas. We also have:

Job description

Job responsibilities

You will work as part of a multi-disciplinary team in a patient facing role;

  • taking responsibility for areas of chronic disease management and undertaking clinical medication reviews to proactively manage patients with complex polypharmacy, providing primary support to general practice staff with regards to prescription and medication queries.
  • helping support the repeat prescriptions system, deal with acute prescription requests, medicines reconciliation on transfer of care and systems for safer prescribing, providing expertise in clinical medicines advice while addressing both public and social care needs of patient in the GP network practices.
  • providing clinical leadership on medicines optimisation and quality improvement and manage some aspects of the quality and outcomes framework and enhanced services, ensuring that the practice integrates with community and hospital pharmacy to help utilise skill mix, improve patient outcomes, ensure better access to healthcare, and help manage workload.
  • playing a pivotal role in improving the quality of care and operational efficiencies so requires motivation and passion to deliver excellent service within general practice.
  • providing leadership on person centred medicines optimisation (including ensuring prescribers in the practice conserve antibiotics in line with local antimicrobial stewardship guidance) and quality improvement, whilst contributing to the quality and outcomes framework and enhanced services through structured medication reviews.
  • undertaking structured medication reviews (SMRs) and follow-up consultations in line with detailed guidance using appropriate clinical decision-making tools to support the delivery of SMRs and clearly record all SMRs within GPIT systems, as well as using appropriate clinical codes to signify the reasons for an SMR.

Person Specification

Qualifications

Essential

  • GPhC registration
  • Be enrolled in or have qualified from an accredited training pathway (currently the Primary Care Pharmacy Education Pathway)
  • Hold or be working towards an independent prescribing qualification that equips the post holder to be able to practice and prescribe safely and effectively in a primary care setting

Knowledge, Skills and Experience:

Essential

  • Demonstrates self-development through continuous professional development activity; working alongside clinical pharmacist to identifying areas to develop
  • Demonstrates an understanding of current educational policies relevant to working areas of practice and keeps up to date with relevant clinical practice
  • Ensures appropriate clinical supervision is in place to support development
  • Reviews yearly progress and develops clear plans to achieve results within priorities set by others
  • Experience of working as part of a multi-disciplinary team
  • Insight into how to evaluate own strengths and development needs, seeking advice where appropriate
  • Experience of providing and receiving complex, sensitive information
  • A willingness to learn new skills and attend training
