PCN Care Co-ordinator

Job summary

In partnership with Southend West Central Primary Care Network (PCN), GP Healthcare Alliance, a federation of GP practices in Castle Point and Rochford, is hosting an innovative service to recruit a care coordinator to support the PCN additional roles team, providing a central co-ordination role for patient care planning across member practices. This role is offered on a full-time basis.

Main duties of the job

This role will require the post-holder to work autonomously, but they will have the support of a multi-professional primary care team made up of doctors, nurses and clinical pharmacists.

The post-holder will work within our multidisciplinary team, providing a central co-ordination role for patient care planning, tosupport delivery of the PCN engagement workstream with the local population by initiating contacts as well as developing and managing relationships across the network.
