Job summary

WGGL Primary Care Network has a fantastic opportunity for two health and wellbeing coaches to join our PCN, to support our 5 GP practices and assist in re-shaping primary care across our communities. One post will cover Wickhambrook and Long Melford surgeries, and the other will cover Guildhall, Clare and Glemsford surgeries, on a full-time basis for 37.5 hours per week. There is an expectation that post holders will cover annual leave at the other surgeries.

Main duties of the job

The main duties include:

  • Coaching and motivating through multiple sessions using personalised goal-setting to implement a patient's personalised health and care plan for long-term lifestyle and behaviour change
  • Identifying frequent attenders and working with patients with lower activation scores to understand and increase their level of knowledge, skills and confidence to self-management their health conditions
  • Pain management psychological element
  • Mental health (well managed)
  • Working with carers
  • Referringto social prescribing link workers and the broader PCN MDT to enable patients to take control of their health and wellbeing
  • Supporting return to work
  • Supporting participation in community activities/exercise to improve mental and physical wellbeing
  • Addressing loneliness
  • Supporting in the recovery from drug and alcohol addiction
  • Lifestyle group education sessions/webinars
  • Responsible for day-to-day planning and personal caseloads
