Job summary

Taunton Deane West Primary Care Network (PCN) is seeking an experienced Digital Coordinator and Administrator to support the delivery of a developing Proactive Care Service. This part-time role (24 hours per week, 12-month fixed-term contract) involves managing IT administration, overseeing service operations, and fostering positive relationships with stakeholders, surgeries, patients, and other services. The successful candidate will ensure all tasks are completed in accordance with established procedures, protocols, and timelines.

Main duties of the job

  • Deliver and manage the developing Proactive Care Service, utilising digital transformation to improve patient care, outcomes, service experience, and cost-efficiency.

  • Enable patients to manage their health and wellness through technology, including communication tools for managing illness, health risks, and medication.

  • Develop and maintain relationships with stakeholders, including internal teams, neighboring practices, patients, and external services.

  • Evaluate and promote the service both internally and externally to ensure continuous improvement and adoption.

  • Manage TDW PCN IT Coordination, including extensive EMIS searches and training for new PCN and Practice employees.

  • Work with digital tools e.g., AccuRx, online access, video consulting to support the transformation of primary care services.

  • Contribute to a population health management approach, leveraging data to improve healthcare delivery.

  • Collaborate with stakeholders to share best practices and drive service development across networks.

About us

Taunton Deane West Primary Care Network is located in Somerset and is formed by three GP Practices, Luson Surgery, Wellington Medical Centre and Lister House.

Within the PCN, the three practices work closely together to improve the quality of care that we provide in alignment with the need of our patient population.

The PCN has a range of additional staff roles, who work alongside Practice staff to help support all of our patients. The team consists of Care Coordinators, Complex Care Nurses, First Contact Physiotherapists, Occupational Therapists, Pharmacists, Mental Health Link Practitioners, Digital Data Coordinators.

The use of technology will give the patient control over their own healthcare utilising information and communication through technology to manage illness, health risks and medicine. The role requires someone to not only deliver the service, but to proactively develop relationships with stakeholders, evaluate the service and help to promote the service internally and externally.

The post holder will manage TDW PCN IT Coordination including management of running extensive EMIS searches and training for new PCN and Practice employees, will work withdata for digital tools to enable transformation of primary care services such as AccuRx, online access, video consulting but not limited to those listed.   • Knowledge of Primary Care
  • Experience of working in a multi-disciplinary setting
  • Able to demonstrate a clear understanding of working with confidential information and an understanding of service user confidentiality
  • Experience of administrative / coordinator duties

Desirable

  • Knowledge/familiarity with medical terminology
  • Understanding of current issues facing the NHS
  • Understanding of health and social care processes
  • Experience in use of databases
  • Interest in digital change, and experience of EMIS, data handling and analysis
  • PR and Marketing skills to aid promotion of the service

Qualifications

  • NVQ Level 2 or equivalent
  • Willing to work towards NVQ Level 3
