Main duties of the job
About the Role
As a Band 6 Commissioning Nurse, you will:
Be responsible for assessing individuals, ensuring the National Framework for NHS Continuing Healthcare and Funded-Nursing Care and other relevant guidance is followed according to the principles and processes. Deliver a high standard of person-centred care.
Work closely with patients and their representatives, hospitals, care providers, and local organisations to support seamless transitions and care provision.
Commissioning and management of safe, cost effective, quality and appropriate care which meets identified health outcomes.
Case management and ongoing review of individuals eligible for CHC or other NHS funded care in accordance with the relevant guidance and legislation.

Clinical leaders and digital leads
Third-sector organisations
