Main duties of the job

The post holder is a Health Coach who acts within their professional boundaries to support people with lower levels of patient activation to develop the knowledge, skills, and confidence to manage their health and wellbeing. The role will contribute to increasing patients physical activity levels, improving diet, and making long-term lifestyle changes.

The role will be to increase patient ability to access and utilise community support whilst providing access to self-management education, peer support and social prescribing.

Health and wellbeing coaches take an approach that considers the whole person in

addressing existing issues and encourages proactive prevention of new illnesses.

The approach will be to appreciate patients circumstances in a non-judgemental manner. This will result in the need for positive communication and negotiation skills that will support the patient personal choices.

The role will be an integral part of the general practice team working in each practice and across the PCN.

Healthier South Wirral Primary Care Network is a group of 6 GP practices in and around the South of Wirral.

Job responsibilities

Primary Responsibilities

The following are the core responsibilities of the PCN health coach. There may be, on occasion, a requirement to carry out other tasks. This will be dependent upon factors such as workload and colleague levels:

a. To work as part of a multi-disciplinary team in a patient facing role to assess and treat patients using expert knowledge.

b. To be responsible for the assessment of new patients with subsequent production and completion of individual care plans

c. To coach and motivate patients through multiple sessions to identify their needs set goals and support them to implement their personalised health and care plan.

d. To provide personalised support to individuals, their families and carers to ensure that they are active participants in their own healthcare and to empower them to take more control in manging their own health and well-being, to live independently and to improve their health outcomes through the following:

e. Providing interventions such as self-management, education and peer support

f. Supporting people to establish and attain goals set by the person based on what is important to them, building on goals that are important to the individual

g. Working with the social prescribing service to connect them to community-based activities which support their health and well-being

h. To be aware of limitations and, as required, be responsible for the referral of patients who require the intervention of other healthcare professionals

i. To work with patients and to understand their level of knowledge, skills and confidence when engaging with their health and well-being

j. To provide support to local community groups and work with other health, social care and voluntary sector providers to support patients health and well-being holistically

k. To identify risk factors and categorise patients into appropriate risk banding

l. To facilitate groups of patients with group consultations to assist patients to work with others to support their own goals

m. To be responsible for the day-to-day planning of personal workloads

  1. To follow departmental policies procedures and guidelines

o. To liaise with the practices and, where practicable, to standardise processes across the PCN

p. To receive referrals and directed patients from triage services and other clinicians

q. To improve the quality and effectiveness of prescribing through clinical audit and education to improve performance against NICE standards and clinical and prescribing guidance

r. To develop yourself and the role through participation in clinical supervision, training and service redesign activities

s. To contribute to a patient safety culture through reporting and investigation of incidents and undertaking proactive measures to improve patient safety

t. To actively signpost patients to the correct healthcare professional

u. To maintain accurate clinical records of all patient consultations and related work

v. To promote health coaching and social prescribing amongst the PCN and networked practices and to support colleagues to improve their skills and understanding of personalised care

w. To review the latest guidance ensuring the practice conforms to NICE, CQC etc.

x. To support in the delivery of enhanced services and other service requirements onbehalf of the PCN

y. To participate in the management of patient complaints when requested to do so and participate in the identification of any necessary learning brought about through clinical incidents and near-miss events

z. To undertake all mandatory training and induction programmes

Secondary Responsibilities

In addition to the primary responsibilities, the health coach may be requested to:

a. Support delivery of QOF, incentive schemes, QIPP and other quality or cost effectiveness initiatives

b. Undertake any tasks consistent with the level of the post and the scope of the role ensuring that work is delivered in a timely and effective manner.

c. Duties may vary from time to time without changing the general character of the post or the level of responsibility.

  • Evidence of a CPD portfolio
  • Experience in a Physical Activity / Sports Development/ Health area within the public, voluntary or private sector.
  • Partnership working.
  • Experience with alcohol or drugs intervention
  • Smoking cessation training (or working towards completion)

  • Experience in health and nutrition work with patients
