Job responsibilities
The Advanced Clinical
Practitioner will be primarily based within the PCN Enhanced Access Hub,
working alongside other ACPs, ANPs, GPs and other health
professionals, delivering care to PCN patients, including home visits.
The
post holder will:
provide advanced clinical skills for patients attending the
service.
have at least 1 years Advanced Clinical Practice experience
be responsible for contributing to the ongoing development of
clinical practice, research and standards of care within the service, including
development of policies and guidelines and contribute to service development to
ensure patients have a seamless pathway.
be expected to practice autonomously, formulate clinical decisions
and treatment pans within local guidelines/policy to manage both acute and
chronic illness.
The
post holder will require excellent communication skills which should include
communicating clearly, sensitively and effectively with patients, families and
professionals. Be able to signpost patients to the most appropriate services
and be enthusiastic to apply acquired knowledge and skills. The
post-holder will have a positive can do attitude, working with
colleagues to deliver safe, kind and effective care.
The
successful post holder will be required to:
Work autonomously, carrying out in-depth clinical assessment of
patients with complex multiple healthcare needs.
Perform advanced skills in assessment, diagnosis, and treatment as
appropriate.
Use specialised theoretical and practical knowledge to solve
problems and make appropriate decisions.
Work collaboratively, maintaining effective communication, with
other professionals and agencies to ensure patient needs are met.
Demonstrate advanced communication skills and provide
psychological support when delivering diagnosis.
Collect, collate, evaluate and report information, maintaining
accurate patient records.
Participate in the development of services and assists the
implementation of change
Provide education and development opportunities for others.
Develop own specialist clinical knowledge and participate in the
development of others clinical knowledge

empathy to enable timely and effective communication with staff, the wider MDT
team and other agencies.

others in a way that suits the situation, demonstrating a high level of
theoretical and practical knowledge.

Responsibilities
Develop
the ability to use reflective practice as required by NMC/HCPC revalidation to
demonstrate how you prioritise

professionalism and trust.
Identify
own personal objectives and development opportunities (and those of other team
members)
through further education and research and audit, therefore continuously
developing this advanced practice role.
Engage in self-directed learning,
critically reflecting to maximise clinical skills and knowledge, as well as
own, the potential of others.
Actively engage and contribute to
the educational development of other staff members and the ongoing educational
strategies within the PCN as directed from the Lead Clinician.
Facilitate collaboration of the
wider team and support peer review processes to identify individual and team learning.
Advocate for and contribute to a
culture of organisational learning to inspire future and existing staff that
supports continuous learning and development, evidence-based practice and
succession planning.
Prepare for and take an active
part in the Appraisal/Revalidation process.
Support the wider team to build
capacity and capability through work-based and professional learning applying
this to practice.
Facilitate medical, nursing and
other Health care professional students on clinical attachments within the PCN.
Resource Responsibilities
All staff

All staff
must ensure they use resources in a manner consistent with organisational
objectives and policies, ensuring that resources are realistic, justified, and
of clear benefit to the organisation.
To ensure
safe and effective use of equipment through staff training, appropriate
maintenance, and repair.

security systems within the workplace according to Practice guidelines

infection control procedures, maintaining work areas in a tidy and safe way and
free from hazards

risks identified.
