Brighter Birkenhead PCN has an exciting opportunity for Care Coordinators to join our multi-disciplinary PCN team, on a part or full-time basis.
A Care Coordinator job is often the single most important role involved in the care of any patient. Supervising interdisciplinary care by bringing together the different specialists whose help the patient may need, the coordinator is also responsible for monitoring and evaluating the care delivered.
We are currently looking for Care Coordinators to cover key PCN projects (Enhanced Access, LARC, Adult ADHD & Diabetes). Dependant on the applicants the role could be split into individual project areas.
Full time and part time are available.
Starting salary will be reimbursed at the indicative Agenda for Change Band 4 entry step point.

Main duties of the job
Care Coordinators provide extra time, capacity, and expertise to support patients in preparing for or in following-up clinical conversations they have with primary care professionals. They will work closely with the GPs and other primary care professionals within the Primary Care Network (PCN) to identify and manage a caseload of identified patients, making sure that appropriate support is made available to them.

Enhanced Access
- Contribute to the monitoring and fulfilment of the clinical and non-clinical rotas.
- Ensure required information is accurately entered onto clinical systems.
- To assist with stock control and ordering of non-clinical items for all services within Operational team (clinical consumables).
- To assist with the process of collecting data for contractual key performance indicators. Collect, collate, process and present operational and clinical information for audit purposes.
- To support the wider team with logging, documenting and investigating service complaints and incidents received this to include reporting for the monthly performance report.
- Effective use of operational software including online rota, medical records, Survey Monkey and relevant others.
Adult ADHD (LEAP)- Responsible for the coordination and management of the LEAP referral process for the PCN, including liaising with clinicians, referrers and patients where required.
- Maintain computerised and manual systems and produce reports as required.
- Process referrals and input data onto computerised record system (EMIS).
- Support the LEAP team with administrative tasks such as telephone calls, email communications, photocopying, processing outgoing and incoming mail.
- Liaise with families within the service; including giving information about our service and booking families onto Information sessions, individual appointments or Managing ADHD Groups.
Long-Acting Reversible Contraception (LARC)
- Manage patient appointments on EMIS Web ensuring appointments are available and clinics are set up appropriately. Maximise clinic potential and limit empty slots.
- Manage a spreadsheet of patient referrals, counselling appointments, fitting appointments, and referring practice. This will support the claim process.
- Manage clinical stock, including reordering, and ensure all equipment is available and clinical room is set up correctly.
- Contact the patients the day before the appointment to ensure attendance.
- Co-ordinate initial appointment with the clinician/patient ensuring patient has sufficient education prior to appointment, signposting where appropriate.
- Ensure appropriate template is used coded correctly supporting clinicians to ensure data is correctly recorded through use of audits and education.
- Liaise with Network Practices to promote services and appointment uptake.
- Facilitate training for team members and ensure training log and schedule is up to date.
- Provide information as and when required to service lead to develop service report.
- Manage waiting lists and management of patient cohort through EMIS or excel spreadsheet.
- Liaise with clinical lead as and when required.
- Ensure achievement of agreed key performance indicators.
Diabetes
- Act as co-ordinator for diabetes 8 care processes programmes.
- Co-ordinating clinics in primary care and liaising with practice/PCN staff.
- Providing admin support to PCN Leadership Team.
- Coordinating and collecting data from primary care to measure various outcomes.
- Undertakes management of the project and gathers information by maintaining project documentation, resources and database.
- To liaise with managers and staff across the diabetes system to support/book educational events.
- To deal effectively via email and telephone with queries.
- To effectively liaise with patients, clinical staff, PCN staff and other professional bodies, acting as a focal point for all general enquiries concerning the work of the clinical team.
