Job summary

MIAA Solutions are advertising the role on behalf of Brighter Birkenhead PCN.

We have an exciting opportunity for Advanced Clinical Practitioners, from a primary care nursing background, to join our multi-disciplinary PCN team, on a part or full-time basis.

The Advanced Clinical Practitioners will work alongside our PCN Lead Nurse to deliver our Enhanced Access Service and develop services at the PCN Hub, including a new Home Visiting Service, respiratory diagnosis clinics and complex respiratory clinics. Applicants must be able to work within our Enhanced Access Service, which operates until 8pm Monday to Friday, and must have access to a car.

Starting salary will be reimbursed at the indicative Agenda for Change Band 8a entry step point. The closing date is 22nd May 2023 and interviews will be held week commencing 29th May 2023.

Main duties of the job

Qualified and highly motivated Registered Nurse or Paramedic, who possesses an MSc in Advanced Clinical Practice/Clinical Masters. The Advanced Clinical Practitioner role works beyond normal professional boundaries as a Registered Nurse/Paramedic and will be working at an advanced level undertaking tasks that have been previously undertaken by medical and/or technical staff. The post holder is identified as a clinical expert and is required to have a high clinical profile ensuring the provision of effective and efficient care. The post-holder would have completed their Non-Medical Prescribing (NMP) training. The post-holder will work within the PCN Enhanced Access Service Hub, including delivering a Home Visiting Service.

Job responsibilities

The Advanced Clinical Practitioner will be primarily based within the PCN Enhanced Access Hub, working alongside other ACPs, ANPs, GPs and other health professionals, delivering care to PCN patients, including home visits.

The post holder will:

  • Provide advanced clinical skills for patients attending the service.
  • Have at least 1 years Advanced Clinical Practice experience.
  • Be responsible for contributing to the ongoing development of clinical practice, research and standards of care within the service, including development of policies and guidelines and contribute to service development to ensure patients have a seamless pathway.
  • Be expected to practice autonomously, formulate clinical decisions and treatment pans within local guidelines/policy to manage both acute and chronic illness.

The post holder will require excellent communication skills which should include communicating clearly, sensitively and effectively with patients, families and professionals. Be able to signpost patients to the most appropriate services and be enthusiastic to apply acquired knowledge and skills. The post-holder will have a positive can do attitude, working with colleagues to deliver safe, kind and effective care.

The successful post holder will be required to:

  • Work autonomously, carrying out in-depth clinical assessment of patients with complex multiple healthcare needs.
  • Perform advanced skills in assessment, diagnosis, and treatment as appropriate.
  • Use specialised theoretical and practical knowledge to solve problems and make appropriate decisions.
  • Work collaboratively, maintaining effective communication, with other professionals and agencies to ensure patient needs are met.
  • Demonstrate advanced communication skills and provide psychological support when delivering diagnosis.
  • Collect, collate, evaluate and report information, maintaining accurate patient records.
  • Participate in the development of services and assists the implementation of change.
  • Provide education and development opportunities for others.
  • Develop own specialist clinical knowledge and participate in the development of others clinical knowledge.

Leadership and Management Responsibilities

  • Be responsible and accountable for your own actions demonstrating effective leadership.
  • Act as a positive role model through provision of strong and effective leadership across professional and organisational teams and boundaries at local level.
  • Pro-actively initiate, and develop effective relationships, fostering clarity of roles within teams, to encourage productive working.
  • Evaluate own practice and participate in multi-disciplinary service and team evaluations with the guidance of the Lead Clinician and the management team.
  • Lead projects that fulfil strategic planning and service objectives, making use of excellent time, people-influencing negotiating skills and management skills.
  • Create a supportive ethos to empower teams to achieve agreed targets, with mutual accountability and contribute to the development of flexible and responsive services.
