Job summary

Mersey Internal Audit and Investigation (MIAA) is advertising the above role on behalf of Central and West Warrington Primary Care Network.

We are a network of 6 practices serving the population of Central and West Warrington. We work together to deliver health care services and improve the health of our population. We are a forward thinking, cohesive and dynamic PCN who like to continually develop and improve.

We are looking for 1 part time Care Coordinator to enhance our existing established team. Experience of working in the healthcare sector is ideal and working with people essential, however full support and training for the role will be provided.

A key aim of this role is to support the increasing Cantonese population of our area and so the ability to speak and translate Cantonese to English and vice versa at good level is required. An understanding of the social and healthcare needs of this population is ideal.

Main duties of the job

Care coordinators play an important role within a PCN to proactively identify and work with people, including the frail/elderly and those with long-term conditions, to provide coordination and navigation of care and support across health and care services.

They work closely with GPs and practice teams to ensure caseloads of patients receive the best care , acting as a central point of contact to ensure appropriate support is made available to them and their carers. This can include booking correct appointment for vaccinations and screening. Support with access to services and navigation of the healthcare system.

This is achieved by bringing together all the information about a persons identified care and support needs and exploring options to meet these within a single personalised care and support plan, based on what matters to the person. This can be by ensuring groups of patients are discussed at MDTS, organising the right people for review by different practitioners or supporting patients to access care or screening that they need.

Care coordinators, review patients needs and help them access the services and support they require.

About us

The successful candidate will be required to work across our 6 practices. However the majority of our patients who speak Cantonese are registered with Great Sankey Medical centre which is a branch surgery of Causeway Medical Centre. The role will involved working closely with the practice teams as well as the existing care coordinator team.

Job responsibilities

  • Help people to manage their needs through answering queries, making and managing appointments, and ensuring that people have good quality written or verbal information to help them make choices about their care.
  • Assist people to access self-management education courses, peer support or interventions that support them in their health and wellbeing and increase their Activation level.
  • Provide coordination and navigation for people and their carers across health and care services, working closely with social prescribing link workers, health and wellbeing coaches, and other primary care professionals; helping to ensure patients receive a joined-up service and the most appropriate support.
  • Work collaboratively with GPs and other primary care professionals within the PCN to proactively identify and manage a caseload, which may include patients with long-term health conditions, and where appropriate, refer back to other health professionals within the PCN.
  • Support the coordination and delivery of multidisciplinary teams with the PCN.
  • Raise awareness of how to identify patients who may benefit from shared decision making and support PCN staff and patients to be more prepared to have shared decision-making conversations.
  • Work with people, their families, carers and healthcare team members to encourage effective help-seeking behaviours;
  • Identify unpaid carers and help them access services to support them.
  • Conduct follow-ups on communications from out of hospital and in-patient services.
  • Support PCNs in developing communication channels between GPs, people and their families and carers and other agencies.
  • Maintain records of referrals and interventions to enable monitoring and evaluation of the service.
  • Support practices to keep care records up to date by identifying and updating missing or out-of-date information about the persons circumstances.
