Mersey Internal Audit and Investigation (MIAA) are advertising the above role on behalf of Central & West Warrington Primary Care Network. The role will be based at Based in Orford Jubilee Hub in Warrington and the successfully candidate will be required to work across our 6 practices.

Main duties of the job
Care coordinators play an important role within a PCN to proactively
identify and work with people, including the frail/elderly and those with
long-term conditions, to provide coordination and navigation of care and
support across health and care services.
They work closely with GPs and practice teams to ensure caseloads of
patients receive the best care , acting as a central point of contact to
ensure appropriate support is made available to them and their carers.
This is achieved by bringing together all
the information about a person’s identified care and support needs and
exploring options to meet these within a single personalised care and support
plan, based on what matters to the person.
This can be by ensuring groups of patients are discussed at MDTS,
organising the right people for review by different practitioners or
supporting patients to access care or screening that they need.
Care coordinators, review patients’ needs and help them access the
services and support they require.
This role is particularly focused on
ensuring that patients are supported to obtain their COVID vaccinations. It will involved working with practices who
are not providing this service to ensure that they have access to the service
provided by CWW PCN.
The initial contract is 6 months. Any extension would be subject to the needs
of the vaccination programme .

We are looking for 1 full time or 2 part time Care Coordinators to
enhance our existing established team.
Full support and development will be provided. Experience of working in the healthcare
sector is ideal and working with people essential. This candidate will need
excellent communication skills and work well as part of a team. Flexibility is a key requirement. The
successful candidate will need to be able to start immediately or within a
couple of weeks. A DBS check and references will be undertaken.
