Mersey Internal Audit (MIAA) is adverting the above role on behalf of Southport and Formby Health Ltd.
We are looking to recruit an Advanced Pharmacist Practitioner (Frailty)
– Clinical Team Leader, to work within our Primary Care Network multidisciplinary healthcare team.
The purpose of the Advanced Practitioner role is to
provide excellent clinical care (via CGA assessment and medication reviews),
alongside Advanced Clinical Management Planning (ACMP), to support the wishes
of patients residing in Care Homes. This is part of the national framework for
Enhanced Health in Care Homes (EHCH) - National
EHCH Framework – an exciting opportunity
for proactive care approach within our elderly population. The role involves
working as the Clinical Team Leader, involved in day to day management of
clinical team members, with support from GP clinical Lead and HR Manager on
site.

Main duties of the job
Provide clinical advice and
prescribing support for other members of EHCH clinical team as needed.
Work alongside the GP Clinical Team Lead and Care Co-ordinator team
leader to manage the clinical workload and ensure DES targets are being met.
To be responsible for day-to-day
management of clinical EHCH staff within the service with support from the GP
Clinical Lead and HR Officer with regards to probationary reviews, team
meetings, annual leave management, annual appraisal, and continuing
professional development.

- Have experience (where possible) of working with frailty/older population demographic.
- Utilise the Comprehensive Geriatric Assessment (CGA) Framework to carry out history taking and examination (within competencies) for Care Home residents.
- Exercise a high degree of personal and professional autonomy.
- Liaise with GP clinical lead re: referral of patients for investigations (within the scope of EHCH team) when necessary and/or liaise with GP Practice colleagues to arrange these.
- Carry out structured medication reviews within the Care Home setting.
- Ensure that all treatments and care provided are based on best practice/evidence-based care.
- Offer clinical leadership and advice to other members of the Enhanced Care Home team including Community Nurse and Physician Associate colleagues.
- Participate in internal MDT (IMDT) twice weekly alongside EHCH team members to triage and prioritise residents referred in for assessments.
- Work with community service providers and other relevant partners as part of monthly external multidisciplinary team (EMDT) for Care Home residents.
- Use digital technology to facilitate medical documentation and communication with other community partners e.g., record keeping within EMIS.
- Identify and/or engage in locally organised shared learning opportunities as appropriate and as capacity allows.
