Job summary

Mersey Internal Audit and Investigation (MIAA) are advertising the above role on behalf of Central & West Warrington Primary Care Network.

  • The role will provide a specialist Paramedic resource for health care professionals and service users, working in collaboration with other members of the Multidisciplinary Team.
  • To work within the community, as an autonomous, accountable, Paramedic Practitioner, in the provision of a holistic approach for individuals including assessment, diagnosis and treatment, to deliver quality patient services.
  • All equipment (medical and IT) and uniform will be provided.
  • You will have a line manager and supervisor. You will work as part of a cohesive and stable team of additional role PCN staff.

Main duties of the job

  • To assess, diagnose, treat, refer or signpost patients/service users who attend surgery with undifferentiated or undiagnosed condition relating to minor illness or minor injury.
  • The post holder will use advanced clinical skills to provide education to service users, promoting self-care and empower them to make informed choices about their treatment.
  • The post holder must own vehicle for home visits with mileage expenses remunerated by submission of a monthly mileage form. Please note it is the post holder’s responsibility to ensure that their car insurance is covered for business use.
  • Our team of 3 Paramedics provide an acute visiting service for housebound patients supported by GPs, minor ailment clinics as required in practices (with appropriate training), Proactive chronic disease review for housebound patients with support from senior practice chronic disease nurses.

Central and West Warrington PCN is a dynamic team of 6 Practices who have a strong and cohesive relationship. We are committed to delivering good quality care to our population of 55,000 patients. Our Paramedics enhance this care by providing proactive and reactive care to our housebound patients and support the activities of the Practices as needs develop.

Helsby Street Medical Centre, Warrington will be the main base for this role and the postholder will also work from all 6 practices within the Primary Care Network (PCN) and MOBILE including patient homes

Job responsibilities

Diagnosing and treating patients presenting with minor illness

  • Triage and treat acutely unwell patients.
  • Triage and treat appropriate patients on home visits.
  • Carry out Avoiding Unplanned Admission reviews
  • Assist with flu and COVID immunisation programmes.
  • Patients with problems needing referral to secondary care should be discussed with the GP supervisor (or GP on call in their absence) before making such referral.
  • Ensure clinical practice is safe and effective and remains within boundaries of competence, and to acknowledge limitations. A GP will be available for supervision conversations at all times.
  • Advise patients on general health care and minor ailments, with referral to other members of the primary and secondary health care team as necessary
  • Works from the surgery and within communities as an autonomous practitioner caring for patients and families.
  • Works as an autonomous practitioner, in accordance with the Health Professions Council. Ensure that personal and professional clinical standards are maintained.
  • To undertake assessment for patients within the community and those attending the surgery, using diagnostic skills and initiation of investigations where appropriate.
  • Prescribe/issue medications as appropriate following policy, patient group directives and local pathways.
  • To formally and informally impart knowledge and skills to colleagues promoting peer review and best practice within the work environment.
  • To communicate at all levels within the team ensuring an effective service is delivered.
  • To maintain accurate, contemporaneous healthcare records appropriate to the consultation.
  • Ensure evidenced-based care is delivered at the highest standards ensuring delivery of high quality patient care.
