Job summary

The post holder will be responsible for implementing and supporting the user of digital technology and associated data sharing in Doncaster in line with the Enhanced Health in Care Homes Framework (EHCH) and model. The post holder will support care homes and general practices across Doncaster fully realise the EHCH digital infrastructure model ensuring effective and efficient use of assistive technology and telemedicine in care homes.

Main duties of the job

  • Work closely with identified elderly care homes in Doncaster to understand how they work and their digital requirements
  • Support the Senior Transformation Manger for Ageing Well develop a plan that describes how to initiate and maximise the utilisation of digital solutions within the care home setting and all other aspects relating to the delivery of the Digital First in Care Homes area of work including:
    • Supporting the implementation of NHSmail in care homes
    • Helping to increase the confidence in digital solutions in the care home setting
    • Championing digital solutions
  • Work closely with the SY Integrated Care Board (ICB) and Voluntary Action Doncaster (VAD) supporting the development of the wider ICB Ageing Well Strategy and to embed the social and digital inclusion agenda across all work areas
  • Be one of the named contacts for care homes in Doncaster regarding their use of digital solutions

Job description

Job responsibilities

  • Embed digital solution utilisation across managers and staff teams in the care home ensuring processes are in place to support data sharing 24 hours/day
  • Support care home staff with digital learning that promotes and embeds the use of digital solutions and basic technology in a focused face to face supportive approach
  • Provide project management to support the delivery of digital activities within a care home ensuring outcomes are met within agreed timeframes
  • Support development and completion of project plans and reports and present these to commissioners and stakeholders as required throughout the project
  • Establish and maintain good working relationships with stakeholders and care homes
  • Act as the liaison and engage with local stakeholders and care homes to better understand what their needs are and support the capture and sharing of good practice, ideas, and activities
  • Work closely with the Digital Community Connector counterpart embedded within VAD
  • Support associated digital transformational projects within the EHCH framework
  • Support the management of the risk and issues tracking mechanism and its proactive resolution and escalation processes
  • Meeting and Agenda coordination and minute taking of meetings as required
  • Delivering excellent customer service at all times
  • Ensure the timely passage of information and respond to enquiries from staff, agencies and the public in a courteous efficient manner

  • Experience of working in Health Care or public sector setting
  • Experience of project management
  • Skilled in the use of IT packages, systems and/or analysing, interpreting, and presenting of data and information
  • Demonstrate the ability to prepare and present reports from complex information, both qualitative and quantitative, to a range of stakeholders

Desirable

  • Educated to degree level in a relevant subject or posses equivalent level qualification or significant experience of working at similar level in relevant subject area
  • Qualification or experience of working in a care home, primary care or community setting
  • Experience of NHS policy/guidance and/or systems
  • IT/digital training qualification or experience
