Job summary

The post holder will support the Federations Service Lead and PCN Manager with the continued implementation of the nationally recommended Personalised Cancer Care Programme (Holistic Needs Assessment & Care Planning, Health and Well-being Event, Treatment Summary, Self-Management including risk stratified remote monitoring and surveillance)

Main duties of the job

Main duties:

Day to day management and organisation of the Macmillan Cancer Care Support Coordinators working across the cancer teams within the PCNs. This will include line management duties, as well as the operational development of the service.

Providing support to cancer specialists when implementing personalised cancer care including assisting with holistic needs assessments and treatment summaries, organising health and well-being events and providing a single point of access, including rapid re-entry into the system for those people identified as having urgent or specialist needs.

Communicating effectively, daily and directly, face-to-face and telephone with a range of individuals on various levels, eg:- patients, relatives, carers, other health and social care professionals and community workers.

Providing general information and support about cancer and cancer services to enable navigation of health and social care and to make choices that are best for the individuals cancer and their life.

Job responsibilities

The post holder will be expected to work with all grades of medical & nursing staff and wider health and social care and community teams including third sector and charity.

Leadership Responsibilities

The post holder will:

Provide day to day direct line management to the Macmillan Cancer Care Coordinators. Duties to include performance management, absence management, annual leave and team recruitment.

Ensure that there is sufficient Co-ordinator cover to complete all essential tasks within the service. This may include providing support and cover where appropriate.

Ensure that the team is compliant in terms of mandatory training and recommended Macmillan Cancer Care Support Co-ordinator learning and development.

Lead Macmillan Cancer Care Support Co-ordinators personal development via regular 1:1s and Personal Development Reviews.

Provide all new Macmillan Cancer Care Support Co-ordinators with a robust local induction programme before being fully integrated into the role and maintain appropriate documentation.

Contribute to ensuring that the Team is managed cost effectively and efficiently within agreed budgets.

Support and facilitate team meetings; minute and distribute outcomes.

Being an excellent role model and leading by example, by demonstrating expertise and maintaining credibility both internally and externally in line with the Federations values and behaviours.

Competencies / Wider Responsibilities

The post holder will themselves undertake the following and lead and support the Macmillan Cancer Care Support Co-ordinators to:

Co-ordinate the care for a defined group of patients assessed by a registered practitioner as having level one care needs for risk stratified supported self-management.

Triage telephone and email enquiries, using a risk assessment framework, initiating an appropriate response according to protocols and individual pathways referring complex decisions to the Team for assessment and review; some requiring tact or diplomacy, prompt action and communication.

Contribute to the holistic needs assessment and the development of individuals care and support plans, including supporting with actions in line with standard operating procedures and protocols.

Co-ordinate and organise appointments, assessments and investigations as needed,

including fast tracking individuals back into the system if required.

Organise and prioritise the designated workload according to identified needs.

Make pre-planned telephone calls to patients to assess on-going needs and to enable a proactive prevention approach.

Co-ordinate the handover with other teams to facilitate safe, effective seamless transition of care between services.

Assist with producing patient treatment summaries.

Assist with the planning and delivery of Health and Well-being events and health and wellbeing information and support.

Demonstrate the ability to recognise and respond appropriately when faced with a sudden deterioration or emergency situation, alerting the team or enabling rapid response.

Communication and signpost to appropriate needs related information and support and encourage self-management where appropriate.

Guide people through the use of self-assessment resources.

Evaluate outcomes of care delivery with the registered practitioner.

Ensure patients and carers know how to contact the service when their condition and needs have changed, including out of hours.

Provide administrative support as needed, including ordering supplies and equipment.

Act as an advocate and facilitator to resolve issues that may be perceived as barriers to care.

Education, Training and Audit

The post holder will lead and support the Macmillan Cancer Care Support Co-ordinators in:

Co-ordinating and providing appropriate learning and development support for the Macmillan Cancer Care Support Co-ordinators.

Identifying individual team members learning and development needs.

Ensuring all members of the team have completed mandatory training in line with the Trust standards.

Promoting healthy lifestyle choices and risk stratified supported self-management as part of consistent and planned aftercare.

Supporting cancer nurse specialists to coach patients and carers to understand side-effects of treatment and signs and symptoms indicating possible recurrence of cancer.

Supporting the delivery of patient and carer training and education, including use of equipment.

Supporting and contribute to audit processes, governance, research, clinical research trials, benchmarking activities, and service development, including using findings and recommendations to change practice and systems.

Developing, managing, and maintaining databases; monitoring the processing of this data.

Analysing and interpreting information and be able to present it different ways so it can be easily understood at a wide range of levels.

Providing support to people affected by cancer, often dealing with highly emotional and sensitive issues and have an ability to manage individual situations effectively.

Demonstrating an awareness of the limits of own practice and knowledge and when to seek support/advice.

Taking responsibility for personal development and education by demonstrating self-directed learning, actively seeking professional development opportunities to enhance practice, knowledge, and role progression.

Acting as an advocate for Macmillan Cancer Support ensuring people affected by cancer know about the full range of resources and services available.

Actively engaging with Macmillan Cancer Support to contribute expertise and experience, support the Macmillan Corporate Strategy.

Acting as an ambassador for Macmillan raising the profile and importance of Personalised cancer care and transformation of service delivery for people affected by cancer.
