Job summary

The successful candidate will be based in Wigan Central & North PCNs. They will be caring, dedicated, reliable, person-focused and enjoy working with a wide range of people. They will have good written and verbal communication skills and strong organisational and time management skills. They will be highly motivated and proactive with a flexible attitude, keen to work and learn as part of a team and committed to providing young people, their families and carers with high quality support.

Main duties of the job

Provide personalised support to children/young people, their families and carers to access community-based activities and support that can help them to take control of their health and wellbeing through co-producing a simple personalised care and support plan and introducing people to appropriate activities, groups and services as described above

Work with appropriate supervision as part of the PCN to manage and prioritise your own caseload, in accordance with needs, priorities and support required by individuals. Refer people to other health professionals/agencies, as appropriate or necessary.

Proactively develop strong links with local agencies to encourage appropriate referrals

Engage with the parents where possible, providing education on supporting their children with their mental health, addressing any behaviour that may be aggravating their condition, and how to work with their own emotions regarding their situation.

Educate non-clinical and clinical staff within PCN MDTs on the community support offer, how and when patients can access it, and the value of non-medical community-based interventions. This may include verbal or written advice and guidance.

As a key member of the PCN Multi-Disciplinary Team, you will provide personalised support to individuals, their families and carers to take control of their health and wellbeing, live independently and improve their health outcomes. You will develop trusting relationships by giving people time and focus on 'what matters to me'. Take a holistic approach based on the persons priorities and the wider determinants of health.

  • Being aware of local support available to young people and their families, such as CAMHS, Wigan Youth Zone, and other young person services.
  • Develop a good knowledge of a broad range of opportunities for young people to engage with their community and become involved in community-based activities.
  • Take a holistic and flexible approach in your work with individuals supporting both social and psychological issues that arise.
  • Provide non-judgmental and non-discriminatory support, respecting diversity and lifestyle choices.
  • Work with individuals to co-produce a simple personalised action plan based on the persons priorities, interests, values and motivations including what they can expect from the groups, activities and services they will be connected to and what the person can do for themselves to improve their health and wellbeing.
  • To work with patients virtually or face-to-face in the local community or in one of our 13 GP surgeries, to support and/or signpost them to local services/organisations, which the patient themselves feel would be of benefit to reach their goals.

  • Manage and prioritise your caseload per the needs, priorities and any urgent support required by individuals and, where appropriate, refer the patient back to health professionals/agencies when the persons needs are beyond the scope of social prescribing.

  • Work closely within the Multi-Disciplinary Teams and with GP practices within the PCN to ensure that the social prescribing referral codes are inputted into clinical systems (as outlined in the Network Contract DES), adhering to data protection legislation and data sharing agreements.

  • Identify risk issues that impact on peoples health or social care needs.
  • Take appropriate action in accordance with the significance of the risk and be consistent with protection procedures, applying protection procedures, and following lone worker procedures.
  • Produce tangible examples of best practice which can be highlighted and shared.
  • Demonstrate effective team working inclusive of all relevant professionals.
  • Report all accidents/incidents, ill health, and equipment and/or environment failures to line managers.
  • Contribute towards audit and data collection as required.

  • NVQ Level 3, Advanced level or equivalent qualifications or working towards
  • Ongoing commitment to learning in relevant areas such as safeguarding and related issues which may impact on young people such as education, housing, relationships, and justice
  • Demonstrable commitment to professional and personal development

Desirable

  • Training in motivational coaching and interviewing or equivalent experience
  • Formal training in Trauma, Mental Health Awareness and a Youth Mental Health First Aid Certificate
