Job summary

An exciting opportunity has arisen to work with Wigan Borough Federated Healthcare as part of a national pilot for early detection of liver cancer.

Wigan Borough Federated Healthcare is looking to recruit a band 6 equivalent registered nurse to join a rapidly expanding Primary Care Network (PCN) in a team that currently includes: Nurse Associates, GP assistants, social prescribers, care coordinators and mental health practitioners.

You will work as part of the Health & Inequalities Team consisting of Quality Nurse Manager, Health & Inequalities Team leader, GP assistants, Nurse Associates and Care Co-ordinators to help provide the provision of General Medical Services, Enhanced Services and work with the Primary Care Network in accordance with the GMS and other practice contracts.

Main duties of the job

You will work within the scope of nursing and care, and within all aspects of the nursing process working in line with NMC standards of proficiency.

Involvement in the safe conduct of research as part of the PCN on the Primary care case finding liver cancer pilot. Identifying, assessing and performing fibro scan on moderate/high risk patients.

You will work across community clinical venues as well as participating in targeted health events aimed at reducing health inequalities.

As a band 6 nurse the postholder is expected to support the team members providing guidance, instruction, direction and leadership and act as deputy by supporting the Quality Nurse Manager leading a team of band 4 GP assistants in the implementation and functioning of the liver pilot awarded to the PCN

You will be an integral part of PCN work-streams, providing high quality, safe effective care to our patient population working both independently and via a multi-disciplinary approach to support patients, their families and carers.

The Primary Care Network (PCN) consists of 13 GP practices and we offer an extension to the GP services in a predominantly community setting utilising spaces within the Voluntary, Community or Social Enterprise organisations. This has helped us in forming strong links between General Practice and the community partners to look after the health and well-being of our local population.

Job responsibilities

You will establish and maintain effective communication, both verbally, written and relationships across the PCN and within multi-disciplinary team.

Maintain professional conduct, be always responsible for own actions and act professionally.

Assess protocols and advise on safety, regulatory and logistical issues for the set up and running of the pilot to ensure the well-being and safety of patients, participants and staff.

Use appropriate computer software to process queries and provide reports on patient accrual.

Participating in update meetings of the project team (both internal and external) to review the progress of specific projects.

Provide participants with specialist information regarding their participation, including explaining all risk factors.

Provide high standards and continuity of care for participants during the pilot, maintaining lines of communication with clinical staff.

Act as a resource to participants, their families and staff from within the clinical area, providing information and support; and to refer to other support agencies where necessary.

Achieve and maintain defined competencies for clinical research to ensure that capability, skill and knowledge are appropriate to the work undertaken.

Undertake, with appropriate training, interventional treatments directly to participants according to pilot protocol and procedures and record the resulting information.

Take and process clinical samples (e.g. venepuncture/cannulation) for studies, coordinate tissue sample collection and dispatch to relevant department or trial centre as appropriate.

Maintain clinical skills as appropriate e.g. vital sign assessment, patient compliance, ECGs, flow rates and fibro scans.

Work at all times as part of the extended multidisciplinary team and maintain excellent links with the staff primary and secondary care

To recognise changes in patients conditions which require the intervention of others and refer on as appropriate?

Support patients when receiving sensitive and life changing health information.

To contribute to the development, implementation and delivery of training packages

To deputise when required for the Quality Nurse Manager nurse as required. This will involve working with staff at all levels and service users. The post will require high standards of integrity, confidentiality and attention to detail.
