Job summary

A fantastic and exiting opportunity has arisen at Wigan Borough Federated Healthcare for a committed, enthusiastic and self-motivated individual to join the team.

As a Primary Care Network Advanced Practitioner, you will work as part of a clinical team, with GPs, Mental Health Practitioners, Nurses, Pharmacists, Pharmacy Technicians physiotherapists, community link workers, care coordinators and Healthcare Assistants across practices in the PCN. The post holder will provide a high-quality, cost-effective service to the patients of Wigan Central and North PCN; diagnosing and prescribing treatments and medication while working within defined procedures and protocols as laid down by the GPs in Care homes.

Main duties of the job

The post holder will be responsible for seeing patients in care homes, who present with a wide range of minor illness, urgent and same day requests for treatment and chronic disease management; providing care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care, and providing proactive care and writing up personalised care plans as per DES and IIF requirements

The successful candidate will be a registered professional with a MSc in Advanced Clinical practice or equivalent, and would have had a minimum of three years post registration with demonstrable experience of using diagnostic skills.

Experience in a primary care setting is desirable, however induction and training in a primary care setting will be given.

Job Summary:

The Advanced Clinical Practitioner is an experienced clinician who can demonstrate a high level of autonomous practice and complex decision making. This is underpinned by Masters level education or equivalent covering the four pillars of advanced practice; clinical practice, leadership and education, research, education.

This senior clinician will provide an expert level of clinical assessment and support to the care and treatment of a diverse caseload of care home providing proactive care. The practitioner will achieve this by using their in-depth knowledge, experience and broad range of techniques, to assess, investigate, interpret, diagnose, treat and refer or discharge patients and create personalised care plans as required in the impact and investment fund (IIF) and Network Contract Direct Enhanced Services (DES).

The Advanced Clinical Practitioner will undertake clinical assessment including history taking and physical examination, form a diagnosis and provide a treatment plan, which may include prescribing. Patient assessment may involve both telephone and face to face consultations.

They will work across care home settings, offering first point of contact at an appropriate place and time. This may include an emergency response to the patient in their own home or other community setting.

The post holder will provide expert clinical and professional leadership and work collaboratively as part of the team to deliver a high quality service that meets the health and social care needs of all care home patients.

Job Responsibilities:

To be professionally and legally responsible and accountable for all aspects of own work, including the management of patients under your care and to ensure a high standard of clinical care for the patients under your management, and those of other staff, by providing expert patient care.

To receive referrals from and make referrals to GPs and other healthcare professionals review the patient and act accordingly, thereby reducing length of time patients wait for intervention and onward treatment, work with the patient, carers and any other parties to create personalised care plans.

To autonomously provide safe and effective highly specialised knowledge and skills, underpinned by theory to instigate and evaluate pathways of care.

To undertake the comprehensive highly specialist assessment of patients, using investigative and analytical skills. The post holder will undertake first line assessment of patients through to discharge or referral, often analysing highly complex facts and situations.

To prescribe the required treatment within the Health Professionals scope of Professional Practice (Independent Prescribing, Patient Group Directions, JRCALC guidelines and Schedule 19 exceptions of the Human Medicines Regulations (2012).

To ensure appropriate, timely and seamless referral to other health and social care agencies through highly developed communication and negotiation strategies.

Actively participate in developing guidelines, policies and protocols as required to support the service delivery. Will be accountable for own professional actions, and will act as the lead specialist role in terms of advanced paramedic practice.

Post holder must maintain professional registration for the duration of their employment and provide evidence to a Clinical Director on annual renewal. Clinicians are also required to abide by the codes of the professional practice as detailed by their professional body.
