Job summary
We are seeking a motivated, innovative and experienced Clinical Pharmacist to join our professional and friendly multi-disciplinary team.
Main duties of the job
The post holder is a PCN Clinical Pharmacist who acts within their professional boundaries, working alongside a team of pharmacists in general practice and within the PCN. They will improve patients health outcomes and the efficiency of the primary care team by providing direct, accessible and timely medicines and expertise including face to face contact with patients.

Network West One Primary Care Network (PCN) consists of 7 GP surgeries in Barking & Dagenham. We cover a population size of over 55,000 patients.

Job responsibilities
Specifically, the Practice-based Clinical Pharmacist will develop and manage medicines optimisation services within Primary Care. They will work as part of a multidisciplinary team to manage medicines on transfer of care and systems for safer prescribing. They will perform face-to-face medication reviews of patients with polypharmacy; especially for those with frailty and/or with multiple co-morbidities. They will undertake face-to-face reviews of patients with specific long-term conditions that fall within their competency. They will provide leadership on quality improvement and clinical audit, as well as managing aspects of medicines safety and antibiotic stewardship. They will contribute to practice financial stability through optimisation of medicines related targets in QOF.

- Registered with the General Pharmaceutical Council (GPhC).
- The ideal candidate will have experience on how to : Undertake clinical medication reviews with patients and produce recommendations for the GP on prescribing and monitoring. To reconcile medicines following discharge from hospitals, intermediate care and into care homes, including identifying and rectifying unexplained changes and working with patients and community pharmacists to ensure patients receive the medicines they need post discharge. See patients with single medical problems where medicine optimisation is required (e.g. COPD, asthma). Make recommendations to GPs for medicine improvements. Answers all medicine related enquiries from GPs, other practice staff and patients with queries about medicines. Provide a telephone advice for patients with questions, queries and concerns about their medicines. Provide education and training to practice teams on therapeutics and medicines optimisation. Implement changes to medicines that result from MHRA alerts, product withdrawal and other local and national guidance. Undertake simple audits of prescribing in areas directed by the GPs, feedback the results and implement changes in conjunction with the practice team. Promotion of evidence-based practice and interventions to prevent the development of infections to practice staff. Reducing gram negative blood stream infections and inappropriate antibiotic prescribing in at risk groups. Demonstrate understanding of the pharmacy role in governance and is able to implement this appropriately within the workplace.
