The PCN Social Prescribing Link Worker plays a vital role in supporting individuals within the Primary Care Network (PCN) to improve their overall health and well-being by connecting them with non-medical community resources and support services. As a key member of the multidisciplinary team, the Social Prescribing Link Worker collaborates closely with primary care professionals, community organisations, and service providers to address the social determinants of health and enhance patient-centered care.

Main duties of the job
Responsibilities:

Assessment and Support:
- Conduct holistic assessments of patients' needs, strengths, and preferences.
- Work collaboratively with patients to develop personalized social prescribing plans.
- Provide ongoing support, guidance, and motivation to individuals in achieving their health and well-being goals.

Community Resource Navigation:
- Identify and maintain an up-to-date knowledge of local community resources, activities, and services.
- Liaise with community organisations, voluntary groups, and support agencies to establish effective referral pathways.
- Signpost patients to appropriate services, activities, and programs that align with their needs and interests.

Collaboration and Networking:
- Build and maintain strong working relationships with primary care professionals, including GPs, nurses, and allied health professionals.
- Collaborate with PCN team members to ensure a coordinated approach to patient care and support.
- Foster partnerships with community organisations, social services, and other stakeholders to enhance social prescribing provision.

Record Keeping and Evaluation:
- Maintain accurate and confidential records of patient interactions and interventions.
- Regularly update patient records in compliance with data protection regulations and PCN guidelines.
- Contribute to the evaluation and monitoring of the social prescribing program, providing feedback and suggesting improvements.
