Job summary
Social prescribing is fundamental to the vision of NHS England and NHS Improvement in delivering personalised care to millions of people through the NHS Long Term Plan. Social prescribing connects people to practical and emotional community support, through social prescribing link workers, who are based in GP practices and take referrals from all local agencies.
It helps people get more control over their healthcare, to manage their needs and in a way that suits them. It can especially help people who:
Social prescribing links them to a range of activities that are typically provided by voluntary and community sector organisations, for example, volunteering, arts activities, group learning, gardening, befriending, cookery, healthy eating advice and a range of sports.
Main duties of the job
To engage, facilitate and support young people in their development, connecting them into activities
to ensure they experience
Reduced feelings of loneliness and isolation – supported to build positive relationships and
improved connections with their community
Improved engagement into a range of provisions and social opportunities – self motivation to
set personal goals and develop aspirations
Improved understanding of risks of their overall wellbeing, manage and reduce personal risk,
increased ability to make positive choices to keep safe and well.

Nottingham City General Practice Alliance is a GP federation that operates to support, connect, and represent General Practices in Nottingham city. We are the lead employer for Primary Care Network (PCN) staff, employing on behalf of our member PCNs.

Social prescribing is a way of engaging primary care patients with a resource which provides support within the local community. In addition, it provides GPs with a non-medical referral option that can align to existing treatments to improve health and wellbeing of patients.
People want to be able to access information and support in a setting and or way that is convenient and familiar to them, delivered by people they trust. As well as ensuring patients access the right care from within the practice, the Social Prescribing Link Worker will refer and connect patients to appropriate clinical and non-clinical services outside of the practice too, often within their local community. Primarily referring and linking patients to the most appropriate service via our digital access platform, Salvie, it will also involve doing so via telephone and occasionally in person.
The role is key in ensuring patients are provided with the best service, information, guidance and direct support to obtain the most appropriate provision for their needs as quickly as possible. The role involves some basic triage and requires a significant degree of managing and prioritising your own workload, in accordance with the needs, priorities and any urgent support required by individuals.
