About us
The Nottingham City ICP brings together all partners who can
influence health and wellbeing. That means being wider than a traditional
health partnership and involving people working in housing, the voluntary
sector, crime and safety and more. The ICP as a place is under pinned by eight
PCNs across the city which represent neighbourhoods covering populations of
between 30-65,000 citizens.
Our
vision is that every Nottingham citizen has access to care and support that
best meets their needs, regardless of their background, circumstances or where
they live in the city. Because Nottingham citizens experience higher levels of
health inequalities than most other places in England, our main focus will be
to target support to people who experience greater levels of inequalities in
the city.
