Job summary
We are seeking a caring and reliable Healthcare Assistant to provide high-quality support to individuals in their own homes. In this rewarding role, you will assist service users with their daily routines and helping them maintain their independence and dignity in familiar surroundings. You will travel between clients' homes, working either independently or as part of a team, ensuring that each person receives compassionate, person-centered care in accordance with their individual care plans.
Excellent communication skills, respect for confidentiality, and a genuine passion for helping others are essential for this position.
Main duties of the job
As a Healthcare Assistant, your primary role is to provide essential care and support to patients in their homes. Your duties are focused on promoting patient independence, dignity, and quality of life.
**Key Responsibilities:**
- Assist with personal care tasks, including washing, dressing, toileting, and providing mobility support.
- Support clients with prescribed exercises, rehabilitation activities, and the use of mobility aids.
- Assist with catheter care, stoma care and end of life care.
- Provide nutritional support, including assistance with meals and hydration.
- Accurately record and report observations and concerns to the appropriate healthcare professional.
- Support clients with medication reminders, as appropriate and in accordance with training and policies.
- Promote independence and encourage self-care whenever possible.
- Maintain patient confidentiality and adhere to safeguarding procedures.
- Contribute to care plans and participate in multidisciplinary team meetings as needed.
- Travel independently between patient homes in the community.

The overall responsibilities of a Home Care Assistant include supporting the provision of
care to clients in their own homes; being alert of their safety; and respecting their dignity. Maintaining skills and knowledge and ensuring that care is delivered according to latest
guidelines and best practice evidence available; ensuring the ongoing safety of clients
through systematic assessment, monitoring and reviewing processes;
Ensuring that clear and accurate records and communication systems relating to clinical and
care delivery are maintained and effectively used acting as a role model for good clinical and
care governance practice.
PERSONAL CARE:
- Dressing and undressing / preparing the service user for Day Care,
or trips out.
- Washing / bathing / showering / shaving / grooming / cleaning teeth.
- Hair care (washing / brushing).
- Toileting and all aspects of personal hygiene.
- Continence management.
- Care of pressure sores (under appropriate nursing supervision).
Getting in and out of bed.
- Assisting with the use of Aids to Daily Living / Rehabilitation Aids, as
required.
- Helping with rehabilitation programmes, as prescribed by Healthcare
professionals.
- Day / evening / night sitting services, as advised.
HEALTHCARE:
- Utilising support, advice, and guidance from the Team Leaders and/or
the qualified nurses within the organisation.
- Assisting the service users who live with long term conditions to
meet their healthcare needs.
DIETARY CARE:
- Preparation of snacks and meals according to the service users likes /
dislikes.
- Assisting with feeding, as required.
DOMESTIC / HOUSEHOLD SERVICES:
- General cleaning duties, to include cleaning / dusting / vacuuming /
polishing.
- Bed-making.
- Clearing refuse and rubbish.
- Laundering / Hand-washing / Ironing / Light needlework, as required.
- Shopping, and the preparation of shopping lists and assistance with
budgeting.
- Light gardening tasks (subject to previous agreement at the Care Plan
stage).
