Post: Domiciliary Care Worker

OVERALL OBJECTIVES
Domiciliary Care Workers to promote service users to remain
independent in their own homes. Our Domiciliary Care Workersdeliver care in the home
and other venues, provide a reliable, personal service which always meets the
individuals personal need and respect confidentiality.
Duties and tasks will include the provision of
personal, social, and enabling care and support to service users within their
own home and in the community.
To assist the Team Leader in the resource planning,
coaching, and mentoring of a team of staff and co-ordinating the daily
provision of care and support for customers to ensure the delivery of a
high quality, responsive and efficient service.
To provide care and support to people with
disabilities to enable them to maintain their tenancies and to achieve their
aspirations for independent living.
The duties will be determined by the individual needs
of the customers being supported. The context in which care and support is provided may
vary according to the users living environment.
Depending upon location, the care and support provided may be to
people with Dementia, people with learning disabilities, neurodiversity, people with mental health related support needs or a
combination of these.
KEY TASKS
CARE & SUPPORT SERVICES
To provide the highest quality
care to service users in their own homes in accordance with their care and support plan,
in person-centred approach.
Identify service users
individual support needs and through consultation and agreement with the
service user (and, where appropriate, their social worker), devise and implement
suitable care and support plans to ensure that identified care and support needs are met.
Encourage and support service
users to live as fully and independently as possible within the local
community, providing information, emotional, organisational, and practical
support and training as appropriate.
Social duties include preparing
and cooking meals, essential cleaning, shopping, bill payment and fire
maintenance.
Personal care includes
washing, dressing, bathing, toileting, and medication monitoring and/or
assistance.
Deliver care and support in a manner
which is sensitive to the service users culture, disability, race, age,
sexuality, marital or civil partnership status, transgender status, or
religion/belief.
Enabling and support may
include such activities as supervising, monitoring, and coaching to empower
service users to be as independent as possible and have control over their own
lives. care workers may also be required to escort or accompany service
users to outings, appointments, or other events.
Participate in staff
development (including supervision), training and performance appraisals as
required.
To carry out risk assessments
as and when necessary due to the changing needs and aspirations of users.
Ensure compliance with
a Fundamental Standards and Innovative Start policies and procedures.
Organise and provide
appropriate care and support for service users preparing to move-on to greater
independence. Contribute to the protection
of service users from abuse and support service users when they are
distressed.
Liaise with housing,
health and social care professionals, benefits, welfare and advocacy agencies,
and other landlords as well as family members, as appropriate.
STAFF
& TEAM WORKING
Work within a team,
supporting colleagues and attending team meetings, recording, and sharing information appropriately and confidentially.
To assist the Team Leader/ Care Coordinators to
achieve service objectives and assist in the development and implementation of
policies, practices, and procedures. Keep up to date with issues
relevant to care & support, welfare benefits and other relevant
legislation.
To undertake all core
training required for this post and other training and professional development
activities as required.
Undertake other delegated
tasks and responsibilities that fall within your capabilities. To be aware of and adhere to ability's current values, policies & procedures. To always comply with the compnaies legal, contractual and regulatory responsibilities in relation to
the above duties.
