Job summary
Assess each residents physical, psychological and social needs and develop up a care plan taking into consideration the lifestyle, gender and background of the people we care for and involve the patient, family and multi-disciplinary team.
Continuously evaluate care needs with other members of the nursing team and ensure that care plans and records are up-to-date and accurate. Continually seek to improve the care delivered.
Give a clear communication of the ageing process and how it affects each persons health and lifestyle. Provide daily visits to assess individual needs and assist service users to maximise their comfort, dignity and independence and create the right environment which supports their physical and mental well-being.

Main duties of the job
Youll support adults with lots of different needs such as learning disabilities, autism, ABI and mental health
Youll support them to flourish in a place they call home. Youll empower the people we support to be ambitious and live fulfilled lives

Youll do activities like cooking, art or listening to music. And get out for movie nights and food outings

Job responsibilities
1. Delegate appropriately but ensure new service users are orientated into
their surroundings and introduced to other service users and staff, and the
appropriate documentation is completed on time.

users needs and within the named nurse system. Ensure the named nurse
system is kept up-to-date.
4. Continuously evaluate care needs with other members of the nursing team
and ensure that care plans and records are up-to-date and accurate. Continually
seek to improve the care delivered to service users.
5. Help service users to understand the ageing process and how it affects
their health and lifestyle. Visit each service user daily to assess their needs
and assist service users to maximise their comfort, dignity and independence
and create the right environment which supports their physical and mental
well-being.
6. Lead a multi-disciplinary shift team, supervising staff in all respects

are adhered to for the safe ordering, custody, storing, disposing and
administration of all medication. Accept responsibility for the safety of
medication in store and on the trolley during dispensing rounds.
9. Manage stocks of all supplies effectively and economically and ensure
the timely order of replenishment stocks.
10. Work co-operatively with activities staff to ensure service users
spiritual and social needs are met.
11. Proactively develop professional and clinical skills in order to enhance
the knowledge and skills needed for safe and effective practice.
12. Contribute to the development of new ideas in nursing and social care in
respect of stimulation for service users to improve their quality of life.
13. Participate in continuous process improvements and make recommendations
for change with regard to clinical and non-clinical policy.
14. Participate in an on-call rota if assessed to be competent.
15. Assist with the investigation of any complaints.
16. Keep the Home Managers informed of any relevant issues.
17. Maintain detailed an up-to-date knowledge of the Mental Health Act.
18. Phlebotomy ensuring required blood tests are up to date.
19. Ordering and checking in medication.
