Main duties of the job

There are around 7 people who use the Outreach service and another 3 who use the Supported Living service. We all live in our own homes and staff come in at various times to support us this can be support with personal care, medication, or social support whatever we need at the time.

We hold our own tenancies in or own our own homes, so you will be a guest in the house for the period of time you are providing support.

For those of us who use the outreach service, we might only have a couple ofhourssupport a week the rest of the time we live our lives independently.

For those of us who use the supported living services, we might have staff all day and night. It is important we have a positive working relationship because we might be spending the whole day together

With experience working in a health or social care setting, you will have an upbeat positive manner, as well as having a genuine passion for supporting people to become a part of their local and wider community.

You will also have experience working with people supported who present behaviours that require intensive support planning and management to ensure their safety and quality of life at all times.

  • Working with MDT teams and participating in complex case reviews for people supported
  • Working with people supported who present behaviours that require intensive support planning and management to ensure their safety and quality of life at all times
  • Proven track record of working in a health or social care setting
  • Previous experience of working across multiple sites
  • Working to budgetary targets and improvement plans
  • Experience of leading teams and managing performance
  • Experience of working to CQC/CI regulations and inspection processes
