Job summary

Due to our expansion in our Clinical Services we are seeking to appoint an enthusiastic and experienced individual to AdultCommunity Nurse. This is a fantastic opportunity to utilise your skills and provide exceptional healthcare to our clients and their families. This is a unique opportunity to create and establish our Clinical services in the area.

Main duties of the job

Youll be working with our Carers and complex care patients in their own homes within the community, ensuring that Carers are suitably trained and competent to deliver the care needed. Youll assess the risks and needs of the patient to devise an individual holistic detailed electronic care plans and that they are regularly evaluated to ensure that patients receive supervised evidence based nursing care in accordance with local contracts.

Job responsibilities

  • Advanced Clinical Support: Provide expert nursing input and assessments for individuals with complex health needs within a social care environment, ensuring care plans reflect current clinical best practices.

  • Care Planning & Risk Management: Lead on developing, reviewing, and implementing personalised care and risk management plans, working closely with care teams to support safe, effective delivery.

  • Health Promotion & Preventative Care: Champion proactive health initiatives, promoting well-being and preventative care strategies tailored to residents needs, including long-term condition management.

  • Multidisciplinary Working: Collaborate with care managers, support workers, GPs, allied health professionals, and external agencies to ensure integrated and coordinated care.

  • Staff Training & Clinical Oversight: Deliver clinical training, supervision, and guidance to care staff, helping to build competence and confidence in delivering complex care.

  • Safeguarding & Compliance: Monitor and promote compliance with safeguarding, infection control, and other regulatory requirements, acting as a key clinical advisor in incident reviews and audits.

  • Clinical Leadership & Quality Improvement: Drive service improvements by identifying gaps in care, initiating quality improvement projects, and contributing to strategic planning within the organisation.

    Essential

    • Current registration to practice

    Desirable

    • Clinical experience including Tracheostomies, ventilators, stoma bags etc
    • Ideally, previous knowledge and/or experience in working in a complex care setting or community setting
    • Excellent communication skills and ability to be able to train clinical and non-clinical staff
    • Submit clinical reports and updates to CCGs regarding their packages as per contractual agreements
    • Maintain clinical updates and evidence of PREP (Post Registration Educational Portfolio)
    • Ideally have leadership skills/experience
    • Driving licence and use of a vehicle for work
