Main duties of the job
We have a wonderful opportunity for an Assistant Case Manager to join our friendly Rehab Without Walls team to support the case management team with individual clients in order to promote independence, maximise quality of life and reduce risk within agreed costs. This is a remote role supporting clients across the south region (Northamptonshire, Bedfordshire, Cambridgeshire, Essex, Surrey, Kent, Hertfordshire)

For over 25 years our Rehab Without Walls team have learned from experience to expect the unexpected! Tales of unusual experiences are passed around, the best becoming RWW folklore. New case managers are regaled with them and soon have their own to add! Between us, we think we’ve developed a pretty formidable (and constantly growing) toolkit of skills, attitudes and actual kit to help us tackle whatever is thrown at us!
For this role you will need to be a driver with the ability to travel, NVQ Level 3 Health and Social Care and experience working as a Support Worker Team Leader
What you'll be working:
- 35 hours a week
- Monday to Friday
- 9am to 5pm
- 1 hour unpaid lunch
What you'll be doing:

Support the case manager in managing and supervising the support team to ensure the appropriate quality of care and rehabilitation for clients

Provide the case manager with timely written and accurate records of supervision for each support worker

Ensure that the goal and action plans relevant to the support workers, and put in place by the case manager, are being achieved in a timely manner

Assist the case manager in the recruitment process of support staff

Where necessary may be required to undertake rota planning for the support staff

Assist with ensuring shifts are covered

Monitor the support workers’ training and ensure that it is current and up to date

Attend regular meetings with the case manager and provide feedback from supervisions

Provide accurate daily timesheets to enable timely billing

Attend supervision with the Senior case manager, at agreed intervals

Ensure that clients have cover for holidays and sickness

Undertake audits on clients with care teams
