Job summary

This role is for a Team Leader - Nights position at Avery Healthcare, located in Wandsworth Common. The successful candidate will be responsible for supporting the home management team in delivering high-quality care, with a focus on creating a safe, caring, and inclusive environment for residents, visitors, and staff.

Main duties of the job

As a Team Leader, your key responsibilities will include leading in the person-centred care planning process, assisting in the smooth transition of resident discharges, monitoring the well-being of each resident, and ensuring safe systems of work across a range of tasks and moving and handling of residents. You will be expected to demonstrate strong leadership skills, effective communication, and the ability to act responsibly in the absence of the manager.

Other responsibilitieswillinclude:

  • Leadingin the person-centred care planning processby implementing,monitoring,and undertaking monthly evaluations, completing care plan audits, developing action plans, andbeing responsible forcompletingthe action plans.
  • Assisting in the smooth transition of any discharges from the home, including documentation, medication, and keeping the residents' next of kin fully informed, where appropriate.
  • Leadingwithmonitoringthewell-being of each resident, being aware of any changeimpactingcare and care planning andensuring that changing needs are met and written records are amended to reflect any variation.
  • Practicing safe systems of work across a range of tasks and moving and handling of residents, by assessing risks and having due regard for personal safety and the safety of residents, visitors, and staff.
