Main duties of the job

As a Team Leader, your focus will be on supporting the home management team in delivering exceptional care. You will be responsible for leading in the person-centered care planning process, assisting with discharges, monitoring resident well-being, and practicing safe systems of work. You will also need to demonstrate the ability to lead staff and act responsibly in the absence of the manager.

Your focus asTeam Leaderwill be tosupport thehome management team in delivering high-quality care with a difference by providing a safe, caring, innovative, open, supportive, and inclusive environment for residents, visitors,and staff.

Other responsibilitieswillinclude:

  • Leadingin the person-centred care planning processby implementing,monitoring,and undertaking monthly evaluations, completing care plan audits, developing action plans, andbeing responsible forcompletingthe action plans.
  • Assistingin the smooth transition of any discharges from the home, including documentation, medication, andkeeping theresidents'next of kin fully informed, whereappropriate.
  • Leadingwithmonitoringthewell-being of each resident, being aware of any changeimpactingcare and care planning andensuring that changing needs are met and written records are amended to reflect any variation.
  • Practicing safe systems of work across a range of tasks and moving and handling of residents, by assessing risks and having due regard for personal safety and the safety of residents, visitors, and staff.
