Job summary

This role as a Team Leader at the Grove Park Care Home in Leeds offers the opportunity to lead a dedicated team in delivering high-quality, person-centred care to residents. You will be responsible for supporting the home management, implementing care plans, monitoring resident wellbeing, and ensuring a safe, caring, and inclusive environment.

Main duties of the job

As a Team Leader, you will focus on supporting the home management team in delivering high-quality care with a difference. Your responsibilities will include leading in the person-centred care planning process, assisting with resident discharges, monitoring resident wellbeing, and practicing safe systems of work. You will need to have an NVQ 3 or equivalent in care, two years' experience in a senior position, and the ability to communicate effectively and lead staff.

Your focus asTeam Leaderwill be tosupport thehome management team in delivering high-quality care with a difference by providing a safe, caring, innovative, open, supportive, and inclusive environment for residents, visitors,and staff.

Other responsibilitieswillinclude:

  • Leadingin the person-centred care planning processby implementing,monitoring,and undertaking monthly evaluations, completing care plan audits, developing action plans, andbeing responsible forcompletingthe action plans.
  • Assistingin the smooth transition of any discharges from the home, including documentation, medication, andkeeping theresidents'next of kin fully informed, whereappropriate.
  • Leadingwithmonitoringthewell-being of each resident, being aware of any changeimpactingcare and care planning andensuring that changing needs are met and written records are amended to reflect any variation.
  • Practicing safe systems of work across a range of tasks and moving and handling of residents, by assessing risks and having due regard for personal safety and the safety of residents, visitors, and staff.
