Job summary

Main duties of the job

As a Team Leader, you will be responsible for leading the person-centred care planning process, monitoring the well-being of residents, assisting with smooth transitions for discharges, and practicing safe systems of work across a range of tasks. You will also be expected to communicate effectively, lead staff, and act responsibly in the absence of the manager.

Your focus asTeam Leaderwill be tosupport thehome management team in delivering high-quality care with a difference by providing a safe, caring, innovative, open, supportive, and inclusive environment for residents, visitors,and staff.

Other responsibilitieswillinclude:

  • Leadingin the person-centred care planning processby implementing,monitoring,and undertaking monthly evaluations, completing care plan audits, developing action plans, andbeing responsible forcompletingthe action plans.
  • Assistingin the smooth transition of any discharges from the home, including documentation, medication, andkeeping theresidents'next of kin fully informed, whereappropriate.
  • Leadingwithmonitoringthewell-being of each resident, being aware of any changeimpactingcare and care planning andensuring that changing needs are met and written records are amended to reflect any variation.
  • Practicingsafe systems of work across a range of tasks andmovingand handling of residents, by assessing risks and having due regard for personal safety and the safety of residents, visitors, and staff.
